Ⅲ期食管癌术前放射治疗临床效果观察

摘　　要：目的 探讨Ⅲ期食管癌术前放疗对手术切除率、淋巴结转移率、残端癌阳性率及远期疗效的影响.方法 252例Ⅲ期食管癌患者,随机分为综合治疗组及单纯手术组各126例.综合治疗组术前给予术前放疗,剂量:4000cGy/4周,放疗后2～3周后手术.结果 2组间手术切除率、淋巴结转移率、残端癌阳性率及术后1年生存率比较,差异具有显著性(P<0.05);2组3年、5年生存率比较无统计学意义.术前放疗未增加手术并发症及手术病死率.结论 Ⅲ期食管癌行术前放射治疗能提高手术切除率及1年生存率,降低残端癌阳性率及淋巴结转移率,3年、5年生存率未有明显提高.Objective To explore the effect of preoperative radiotherapy on the resection rate,rate of node metastases,positive stump rate and long-term efficacy for esophageal cancer of Ⅲ stage.Methods All of the 252 cases of esophageal cancer of Ⅲ stage were randomly divided into 2 groups:126 cases have received preoperative radiotherapy 2～3 weeks before operation,dose:4000cGy/4weeks.The others received the operation without preoperative radiotherapy.Results There were significant difference for resection rate(P<0.05) and rate of node metastases(P<0.01) between two groups.Difference of positive stump rate(P<0.01) and 1-year survival rate(P<0.05) has been found between two groups.But there was no difference of 3 and 5-year survival rate between two groups.There were no difference of complication and death rate of surgery between two groups.Conclusion Preoperative radiotherapy for esophageal cancer of Ⅲ stage can significantly improve the resection rate and 1-year survival rate.It also can reduce the rate of node metastases and positive stump rate.But it can't improve the 3-and 5-year survival rate.
