头项针治疗血管性痴呆临床研究

摘　　要：目的观察头项针治疗血管性痴呆(vascular dementia,VD)的临床疗效,为临床治疗VD提供规范、有效的治疗方法。方法60例VD患者随机分为2组,针刺组30例应用头项针治疗,取头部穴额三针、顶三针、颞三针,项部穴项三针,采用毫针刺法,诸穴得气后,做快速捻转手法30s,留针30min,每日1次,每周休息1日,治疗28日。喜得镇组30例,予喜得镇,每次1mg,日3次,服药28日。2组评定疗效,同时测定治疗前后简易精神状态量表(MMSE)、日常生活功能评定量表(ADL)积分及中医证候治疗前后的积分。结果①针刺组有效率为60%,喜得镇组有效率为40%,针刺组疗效高于喜得镇组,2组比较差异有统计学意义(P<0.05)。②2组治疗后MMSE量表积分均有所增加,2组比较差异有统计学意义(P<0.05)。2组治疗后ADL量表及中医证候积分均有所下降,2组比较差异有统计学意义(P<0.05)。结论头项针治疗VD疗效确切,且能改善患者的智能状况、日常生活能力,疗效优于喜得镇,应进一步在临床上推广应用。Objective To investigate Vascular dementia treated by head-acupoint,supply standard and valid treatment for clinical practice.Methods All 60 patients, according to history syndrome and physical examination and the standard of DSM-IV and Haschanski about Vascular dementia disease, were divided two groups: Head-Neck Acupuncture group and Drug group.The main points of Head-Neck Acupuncture group was Qianting Touwei(double) Baihui Sishencong Shuaigu Fengfu Fengchi (double). Patients in Medicine group take Hydergine 1mg 3 times a day.The course was 28 days. At the same time the scores of MMSE, ADL,TCM syndrome were measured before and after treatment.Results 1. The effective rate is 60% in the Head-Neck acupuncture group, 40% in the medicine group. There was a significant difference between the two groups (P<0.05). 2. The scores of MMSE in two groups are all increased after treatment, the changes in Head-Neck Acupuncture group was superior to that in Drug group(P<0.05). The scores of ADL and TCM syndrome in two groups are all decreased after treatment, the changes in Head-Neck Acupuncture group was superior to that in Drug group(P<0.05).Conclusion Head-Neck acupuncture can be regard as a effective and convenient way to treat VD, effective rate of the Head-Neck acupuncture group is much higher than medicine group, it is worth to used for clinical practice.
