遂道自闭式小切口非超乳人工晶体植入术28例临床应用

摘　　要：目的 :采用遂道自闭式小切口非超乳白内障摘除人工晶体植入术 ,治疗白内障 ,减小术后角膜散光 ,减轻组织损伤 ,减少并发症 ,较快恢复视力 ,降低手术费用。方法 :以 12点为中心 ,距角膜缘后 1.5mm～ 2 .0mm ,做一弦长 5 .5mm～ 6 .5mm反眉状切口 ,深度为 1/ 2巩膜厚 ,用 11号尖刀片向前潜行分离巩膜遂道 ,至角膜透明层内 1mm处刺入前房 ,前房内注入粘弹剂后截囊针环形撕囊 ,水分离皮质与核 ,并转动核 ,使核周皮质尽量分离 ,扩大内口 ,以注水圈匙娩出晶体核 ,冲净残留皮质 ,前房及囊袋内注入粘弹剂 ,植入人工晶体于囊袋内 ,缩瞳 ,吸出粘弹剂 ,整复切口 ,结膜下注射庆大霉素、地塞米松 ,单眼包扎 2 4h。结果 :术后 1d～ 2周视力≥ 0 .5者 ,2 4眼 ,占 80 %。视力 <0 .5者 ,6眼 ,占 2 0 %。术后三个月矫正视力 >0 .8者 ,为 86 .7% ,散光度数 1.2 5± 0 .75。结论 :采用遂道自闭式小切口非超乳白内障摘除人工晶体植入术 ,治疗白内障 ,术后角膜散光小 ,视力恢复快 ,组织损伤轻 ,并发症少 。Objective:To introduce self-sealing sclera tunnel incision for the non-phacoemulsification and intraocular lens implantation.Method: On 30 eyes,self-sealing incision for the non-phacoemulsification and intraocular lens implantation was performed with the center of incision at the 12 o’clock point,the position of incision at 1.5mm-2mm behind the corneal margin,length 5.5-6.5mm.Result:The uncorrected visual acuities of postoperation lday to 2week≥0.5 were in 24 eyes(80%),<0.5 were in 6 eyes,the visual acuities with spherical correction,>0.8 is 86.7%,the degree of astigmatism is 1.25±0.75.Conclusion: This method used in the treatment of cataract has the advantages of less astigmatism,sight recovering quickly,wild tissue injury,less complication,less cost.
