鼻咽癌组织中P53和PCNA表达与临床分期、VCA/IgA、EA/IgA、放射敏感性和预后的关系

摘　　要：背景与目的:P53和增殖细胞核抗原(proliferatingcellnuclearantigen,PCNA)与恶性肿瘤的发生发展和预后有一定关系。有关P53和PCNA与鼻咽癌临床分期、VCA/IgA、EA/IgA、放射敏感性和预后关系的文献报道不多。本研究检测鼻咽癌组织中P53和PCNA的表达强度,并探讨其与上述临床指标的关系。方法:用免疫组化的方法检测80例单纯放疗的鼻咽癌组织中P53和PCNA的表达强度,通过卡方检验分析其与临床分期、VCA/IgA、EA/IgA、放疗剂量36Gy时鼻咽肿物和颈淋巴结的消退情况及随访5年生存率的关系,探讨其临床意义。结果:80例单纯放疗鼻咽癌组织中P53阳性表达率92.5%,PCNA阳性表达率100%,PCNA表达强度与放疗中剂量达36Gy时鼻咽肿物和颈淋巴结的消退情况Mr和Pr有差异显著性(0.05>P>0.01),而P53则没有差异显著性(P>0.05);P53和PCNA表达强度与临床分期、VCA/IgA、EA/IgA、以及三组生存期<3年、<5年、≥5年均无差异显著性(P>0.05)。结论:P53和PCNA与鼻咽癌的发生有一定相关性;PCNA与鼻咽癌放射敏感性有关,而P53则与之无关;P53和PCNA与鼻咽癌的临床分期、VCA/IgA、EA/IgA以及预后无关。BACKGROUND &OBJECTIVE: P53 and proliferating cell nuclear antige n( PCNA) relate with tumorigenesis, development, and prognosis of malignant tumors. This study was to detect expression of P53 and PCNA in nasopharyngeal carcinoma (NPC) tissue, and discuss their relation of p53 and PCNA expression with clinica l stage, VCA/IgA, EA/IgA, radiation sensibility, and prognosis. METHODS:Immunohi stochemistry was used to detect P53 and PCNA expression in NPC tissue of 80 pati ents received radiotherapy alone. Relation of P53 and PCNA expression with clini cal stage, VCA/IgA, EA/IgA, extinction of nasopharygeal tumor and neck lymph nod e when radiation dose was 36 Gy, and 5-year survival rate was analyzed by Chi-square test. RESULTS: Positive rate of P53 in NPC tissues was 92.5%, and that o f PCNA was 100%. Expression intensity of PCNA significantly related with extinc tion of nasopharygeal tumor, and neck lymph node when radiation dose was 36 Gy, but P53 had no significant relation with it. Expression intensities of P53 and P CNA had no significant relation with NPC clinical stage, VCA/IgA, EA/IgA, and 5-year survival rate. CONCLUSIONS: P53 and PCNA relate with NPC occurrence. PCNA relates with NPC radiation sensibility, but P53 doesnt. P53 and PCNA have no relation with NPC clinical stage, VCA/IgA, EA/IgA, and prognosis.
