p53等四种基因表达水平与食管癌病人术后生存期的关系

摘  要:目的 :研究 p5 3、PCNA、C erbB 2和nm2 3基因表达水平与食管癌切除术后病人生存期的关系。 方法 :按术后 2年内死亡 (对照组 )和长期生存 5年以上 (生存组 )分组 ,随访食管癌切除术后病人 6 3例 ,用免疫组织化学技术对手术标本进行四种基因检测。结果 :生存组和对照组中的 p5 3、PCNA、C erbB 2基因表达均较强烈 ,与食管癌恶性程度有关 ;生存组的nm2 3基因阳性表达 (2 7/ 31) 87 1%明显高于对照组 (9/ 32 ) 2 8 1%。结论 :本组研究病人的p5 3、PCNA、C erbB 2表达与生存期长短无明显相关性 ;nm2 3的强烈表达与食管癌病人生存期明显相关 (P <0 0 1) ,对食管癌的转移和复发有较强的抑制作用 。Objective: To investigate the relationship between the levels of gene expressions of p53, PCNA, C cerbB 2 or nm23 and the survival stage in patients with esophageal carcinoma after operation. Methods: The patients were divided into two groups: death within 2 years (control group) and long term survival for more than 5 years (survival group) after operation. Sixty three post esophagectomy patients were followed up. The specimens of esophagectomy were examined by immunohistochemical method for the above 4 genes. Results: p53, PCNA and C erbB 2 genes expressions were all clearly overexpressed both in the control group and survival group. They were associated with the severity of esophageal carcinoma. The positive expression of nm23 gene in the survival group (87.1%; 27/31) was significantly higher than that in the control group (28.1%; 9/32; P <0.01). Conclusion: The expressions of p53, PCNA and C erbB 2 are not clearly correlated with the survival stages. The strong expression of nm23 genes is clearly associated with the survival stage in patients with esophageal carcinoma ( P <0.01), and they have strong inhibitory effect on metastasis of esophageal carcinoma and relapse. It has some significance in judging prognosis in patients with esophageal carcinoma after operation.
