垂直式小梁切除术治疗青光眼的疗效观察

摘　　要：目的 :探索治疗青光眼的新方法 ,以提高治疗青光眼的手术成功率 ,减少术中及术后并发症。方法 :采用三角形板层巩膜瓣 ,切除小梁组织 1.0～ 1.2mm ,直视下做虹膜根切 ,三角形板层巩膜瓣顶缝合 1针 ,连续缝合球结膜。术后处理同一般小梁切除术。结果 :87例 10 3眼行垂直式小梁切除术 ,术中无并发症 ;术后近期前房有轻度炎症反应 ;前房出血 2眼 ;1度浅前房 2 8眼 ,2度浅前房 9眼 ,未见脉络膜脱离。经术后随访 6月～ 2年 ,眼压≤ 2 .74kPa(2 1mmHg)为 10 0眼 ,眼压控制率为 97%。结论 :垂直式小梁切除术治疗青光眼 ,与标准小梁切除术相比 ,手术操作简单 ,术中、术后并发症少 ,治疗效果好的优点。Purpose To explore a new method to treat glaucoma in order to promote the success rate of glaucoma operation and reduce complications during and after the operation.Methods Using triangular lamellar and sclera flap and making a 1×4mm vertical excision of behind sclera.Then make iridectomy under direct view;suture 1 point on the top of triangular lamellar sclera flap and suture bulbar conjunctiva continuously.The post-operation treatment is the same as that of trabeculectomy.Results 103 eyes of 87 patients were taken vertical trebeculectomy,there was no complication during the operation.After operation,there was slight inflammation in the anterior chamber in all eyes and anterior chamber hemorrhage in 2 eyes and 1 dgree shallow anterior chamber in 28 eyes and 2 dgree shallow anterior chamber in 9 eyes .But no choroids detachment was observed.During the 6 months to 2 year's following-up study,there were an intraocular pressure lower or quit 2.74Kpa in 100 eyes.The control rate of intraocular pressure is 97%.Conclusion Comparing with standard trabeculectomy operation,the vertical trabeculectomy is simple,has less complications and better effect.
