内镜下联合治疗116例血吸虫相关肝硬化食管静脉曲张出血临床分析

摘　　要：目的观察套扎法(endoscopic variceal ligation,EVL)与硬化法(endoscopic variceal sclerotherapy,EVS)联合治疗血吸虫相关肝硬化食管静脉曲张出血(esophageal variceal bleeding,EVB)的疗效。方法首先采取EVL,每例套扎1～6环,在套扎点再进行EVS巩固治疗,每次硬化4～6点,每例都在1周后复查同时给予再次EVS,但剂量适当减少,2周后根据情况再行EVS+EVL,该联合治疗法至曲张静脉消失或变细。结果56例单纯急诊EVL共70例次治疗,53例即时止血,急诊止血率95%,中转手术2例,死亡1例。60例EVL+EVS联合治疗患者共行81例次套扎、134例次注射,急诊止血率93%,中转急诊手术3例,死亡1例。结论EVL和EVS+EVL对EVB均是安全有效、副作用少的治疗方法,合并酒精性肝病或者病毒性肝炎的血吸虫相关肝硬化患者出血止血效果差。【Objective】To observe the therapeutic effect of EVL plus EVS for EVB correlated with schistosomiasis. 【Methods】After EVL of 1～6 bands, 4～6 spots were performed EVS for advanced therapy, one week later, one more EVS be given for recheck, but the dosage of injection decreased, according to the condition of patients EVS+ EVL would be given or not in two weeks. 【Results】Among 56 patients treated with EVL alone 53 stop bleeding immediately, 2 transfered to operate, 1 died, the rate of emergency haemostasis was 9...
