摘  要:目的 :通过对皮瓣组织中毛细血管内皮细胞表面的ICAM - 1表达量的观察 ,反映皮瓣局部的炎症反应程度、白细胞浸润情况 ,了解血小板激活因子受体拮抗剂BN5 2 0 2 1对皮瓣局部的炎症反应的拮抗作用和血小板激活因子 (PAF)对鼠任意皮瓣成活能力的影响。方法 :使用BN5 2 0 2 1对皮瓣局部进行处理 ,然后于术后 3、6、9h将皮瓣取下 ,分区段进行免疫组化检查 ;并于 7d后观察皮瓣存活面积。结果 :与对照组相比 ,BN5 2 0 2 1处理组皮瓣中的血管内皮细胞表面ICAM - 1的表达明显减少 ;术后 7d皮瓣成活面积 ,处理组明显大于对照组 ,P <0 .0 0 1。结论 :在鼠背部任意皮瓣移植时 ,BN5 2 0 2 1可以降低皮瓣局部的白细胞浸润程度 ,抑制炎症反应 。Objective: Platelet-activating factor ( PAF) is an extremely potent lipid-inflammatory mediator. The purpose of this study was to evaluate its possible effect on flap inflammatory reaction and investigat e the mechanism of platelet-activating factor antagonism in the surviva l of ran dom flaps, using a SD rat dorsi random flap model. Methods: A lipophilic platelet-activating factor receptor antagonist (BN52021) was adm inistered in a single dose to treat flaps by a local percutaneous injection rout e 30 min prior to the operation. 3、6 and 9 h after the transplantation, full-t hi ckness flap hiopsies were collected sequentially and analyzed immunohistochemi cally for ICAM-1 expression. Results: The platelet-act i vating factor receptor antagonist was found to attenuate ICAM-1 expression s igni ficantly in the treated flaps. During transplantation, pla telet-activating factor antagonist is able to diminish, directly or indirectly, the accumulation of acute inflammatory cells in random flaps.
