自体富含血小板血浆复合倍骼生用于牙槽骨增量的临床研究

摘　　要：目的:评估自体富含血小板血浆复合倍骼生用于牙槽骨骨量不足的牙种植术的临床效果。材料和方法:对17例存在骨量不足的种植牙患者,其中3例行上颌窦提升术,14例种植体周围骨缺损,植入36颗种植体,采用富含血小板血浆复合倍骼生恢复种植体周围的骨缺损。采用ELMD500富血小板血浆制备系统。术前抽取患者肘部静脉血10ml,先以5600rpm离心10分钟分离出贫血小板血浆,再以2400rpm离心10分钟分离出红细胞成分,最后获得富血小板血浆常温备用。使用时取凝血酶100u加入10%Cacl22ml,然后加入5mlPRP摇匀后加入倍骼生,形成一种凝胶状的混合物,作为植骨材料用于上颌窦提升和GBR手术。结果:17例病人中,愈合期无一例出现感染,有3例出现伤口裂开,裂开后局部使用盐酸米诺环素处理伤口,伤口延期愈合,其余病员均一期愈合。二期手术翻瓣暴露植骨区,植入前宽度平均宽度为3.99+1.86mm;二期手术时牙槽嵴宽度为7.37+0.91mm。牙槽嵴顶部宽度平均增加3.36mm,二期手术时没有种植体脱落。植骨前与二期手术时牙槽骨宽度p<0.05,植骨后和二期手术时相比p>0.05。6～8个月后X线检查提示种植体周围的骨结合好,修复效果好。结论:富含血小板血浆复合倍骼生可以较好的修复种植体周围的骨缺损,无需另开术创切取自体骨,创伤小,效果可靠。Objective:To evaluate the bone regeneration effect due to platelet rich plasma combined perioGlas in reconstruction of perioimplant bone defect.Materials and Methods:A total number of 24 implants were placed in 17 patients(11 males,6 females,with an average of 33.4 years),3 of them perform maxillary sinus augumentation,14 perform GBR for peri-implant bone defects.platelet-rich plasma in combination with PerioGlas were used as bone graft material.X-ray examination were carried out prior to and after operation,and 6months after operation.In GBR cases,thickness measurement were carried out prior to GBR,after GBR and 6month after when II phase operation performed.Results:No implant lost in observe period.6-8months after first stage surgery,X-ray and clinic examination shows good osseointegration achieved.The Thickness of bone before GBR were 3.99+ 1.86mm;7.55+ 1.04mm after GBR,and 7.37+0.91 mm 6months after when II operation perfomed.The thickness increased 3.36mm average(p<0.05).Conclusion:This study suggests that platelet-rich plasma in combination with PerioGlas is a reliable material for GBR and maxillary sinus augumentation.
