摘  要:目的:探讨用23.4%高渗盐水治疗颅内高压时对患者心功能的影响。方法:对20名昏迷的、有侧脑室引流指征的脑出血患者进行共46例次降颅内高压治疗。静脉推注23.4%高渗盐水30ml。用药前至用药后6h内,持续监测颅内压(ICP)、中心静脉压(CVP)、血氧饱和度(SaO_2)、心率(HR)、呼吸频率(R),并观察患者肺部罗音的变化,行胸片检查。结果:用药前后CVP、SaO_2、HR、R保持稳定(P>0.05),未见急性心功能不全表现。结论:一次性使用23.4%高渗盐水30ml治疗颅内高压,未见引起急性心功能不全发作。Objective: To determine the effects using 23.4% hypertonic saline in treatment of intracranial hypertension on the cardiac function. Methods: We present 20 comatose patients, who had cerebral hemorrhage and the indications for the use of lateral ventricular puncture, and a total of 46 episodes of intraeranial hypertension to treat with the intravenous bolus administraetion of 30ml of 23.4% saline. We monitored continuously intracranial pressure(ICP), central venous pressure (CVP), oxygen saturation(SaO_2), heart rate(HR) and respirations(R), noticed the change of rales from initiation of treatment to 6 hours flowing it, Chest X-ray was done before and after treatment. Results: There was no significant difference in CVP, SaO_2, HR and R before and after treatment(P>0.05). Any clinical evidence of acute heart failure was not found. Conclusions: Any clinical or laboratory evidence of acute heart failure was not found, while using 23.4% hypertonic saline once in treatment of intracranial hypertension.
