硫喷妥钠联合机械通气治疗老年癫痫持续状态的临床探讨

摘　　要：目的评价硫喷妥钠联合机械通气抢救老年患者癫痫持续状态的疗效。方法45例老年患者依据临床特征、脑电图确诊为癫痫持续状态,入院后立即行硫喷妥钠1.0g加入0.9%氯化钠溶液中,使用微量泵3～8mg/min静滴。同时采用气管插管机械通气,根据血气分析合理选择呼吸模式和调节呼吸机参数,加强呼吸道管理。结果45例于5～30min内抽搐全部控制,3～24h内意识转清,生命体征、血气分析均恢复正常。结论使用硫喷妥钠联合机械通气控制老年患者癫痫持续状态快速、高效、安全。Objective To evaluate the effect of combining thiopental with artificial ventilation on aged status epilepticus, for exploring an effective way for status epilepticus clinicaltherapy. Methods The 45 cases of aged status epilepticus patients were final diagnosed by clinical examination and eletro encepalograph (EEG) . The first time after in-hospital, 5%thiopental was intravenous injected by micro-infusion pump, meanwhile, trachea intubation and artificial ventilation were adopted. The optimal ventilation patterns and parameters were set up by blood gas analysis monitoring. Result The tic of all 45 cases was controlled within 5～30min after treatment. The mind of patients were recovered. The vital signs and blood gas analysis returned to normal. Conclusion Combining thiopental with artificial ventilation is a fast, effective and safe way for aged status epilepticus clinical therapy.
