外固定架在不稳定骨盆骨折急诊救治中应用

摘　　要：目的探讨急诊外固定架治疗不稳定骨盆骨折的救治方法和疗效。方法采用外固定架于受伤24h内行急诊固定不稳定骨盆骨折8例,包括交通伤5例,高处坠落2例,塌方挤压伤1例。结果8例中7例单纯应用外固定架治疗,平均随访7个月,按照Matta氏评定标准,优良率达87·5%。结论对于合并有其他脏器损伤和失血性休克的不稳定骨盆骨折,急诊应用外固定架能简单、迅速地完成骨盆骨折的固定,恢复骨盆的稳定性,并可作为最终的治疗手段。Objective To explore the methods and clinic effects of unstable pelvic fracture treated with external fixator.Methods Eight patients with unstable pelvic fracture,including traffic injury(n=5),injuries following falling from high places(n=2),and crush injury by collapse(n=1),were treated external fixator within less than 24 hours after injury in the emergency rooms and followed up for 7 months(4 months^1.5 years).Results Seven patients were treated only by external fixator and I C2 type patient underwent internal fixation 10d after the external fixation.Seven of the 8 patients(87.5%) got excellent results according to the Matta evaluation system.Conclusion A simple procedure,external fixation can be used quickly to fix the unstable pelvic fracture,while keeping the integrity and stability of the pelvis for the patients with other major organ injuries or hemorrhagic shock,and it can serve as a last fixation device too.
