摘 要:目的通过比较腹腔内大小剂量化疗的疗效,同时观察肿瘤局部及外周血中化疗药的药代变化,比较两种疗法的疗效优劣。方法建立黑色素瘤移植鼠模型;分别观察比较大(12mg/kg)小(4mg/kg)剂量顺铂行腹腔内注射后荷瘤鼠的腹膜瘤结节减少情况,并取鼠大网膜及外周血测定含铂量。结果在动物实验中,与空白对照相比,大小剂量顺铂组瘤结节数量均明显减少;用药后第3、6天时,大剂量顺铂组瘤结节为(126.00±13.68)、(168.17±15.14),而小剂量顺铂组瘤结节分别为(91.17±13.83)、(92.00±11.33),后者明显少于前者,且(P<0.01);用药后第6天时小剂量顺铂组大网膜内含铂量为(997.79±280.08)ng/g,明显高于大剂量顺铂组的(471.29±280.08)ng/g,(P<0.05);小剂量顺铂组外周血中含铂量在用药后均明显低于大剂量顺铂组,(P<0.05)。结论腹腔内化疗能明显减少小鼠腹腔内瘤结节的形成;与一次大剂量化疗相比,小剂量连续化疗疗效更好。Objective It is a effective way for intraperitoneal chemotherapy to cure peritoneal neoplasm, but the researches about comparison between drugs in high-dose and in low-dose have been rarely reported. Through comparing of the effects of intraperitoneal chemotherapy in high-dose with in successive low-dose and observing of pharmacokinetic changes of cisplatin absorbed in tumor and in blood, we appreciate the curative effect of two kinds treatments.Methods Peritoneal transplant carcinoma model with C57BL mice were set up by inoculating 8 x 105 B16 melanoma cells per mouse, which were treated with cDDP in high-dose(12 mg/kg) or in successive low-dose(4 mg/kg). The numbers of peritoneal carcinomatous nodes were counted and the concentrations of platinum in blood and epiploon of mice were detected by atomic absorption spectrophotometry.Results In animal experiment, the number of melanoma nodes of mice in high-dose and successive low-dose cDDP group is much less than one in control group in the 2nd,3rd and 6th day(P<0.05 ). In the 3rd and 6th day, the numbers of melanoma nodes in high-dose cDDP are separately 126.00±13.68 and 168.17±15.14, but the ones in successive low-dose cDDP are separately 91.17±13.83 and 92.00±11.33. The latter is much less than the former (P<0.01). Although the concentrations of platinum in epiploon of mice in successive low-dose cDDP is(997.79±280.08)ng/g in the 6th day, it is higher than one in high-dose cDDP(471.29±280.08)ng/g, and( P<0.05).The concentrations of platinum in blood of mice in successive low-dose cDDP is much lower than one in high-dose cDDP, (P<0.05).Conclusion The number of melanoma nodes of mice can be obviously reduced by intraperitoneal chemotherapy, and the curative effect on mice with peritoneal transplant carcinoma in successive low-dose cDDP is much better than the one in high-dose cDDP.
