促进化脓性腹膜炎手术康复措施的探讨

摘　　要：目的探讨促进化脓性腹膜炎手术后胃肠功能的恢复,提高切口甲级愈合率的措施。方法总结分析2003年～2005年间通过加强围手术期处理施行手术治疗的胃十二指肠溃疡穿孔腹膜炎和化脓性阑尾炎并腹膜炎587例,其中2003年1月至2004年6月282例为A组,主要处理措施包括:术前预防性抗生素的使用、术野铺粘贴性手术薄膜、仔细的手术操作、腹腔的确切冲洗、碘伏冲洗切口;2004年7月至2005年12月305例为B组,B组在按A组处理的基础上加用术尔泰冲洗腹腔,可吸收线连续缝合腹膜切口等措施。结果在胃肠功能恢复方面B组明显优于A组(P<0.05)。切口甲级愈合率A组88.65%,B组91.15%;切口感染率A组4.97%,B组3.93%。结论通过加强围手术期处理及改进手术处理,有促进化脓性腹膜炎病人胃肠功能恢复、提高切口甲级愈合率的作用,有实用价值。Objective In order to discuss the promotion of suppurative peritonitis surgery the stomach and intestines function restoration, enhances the margin a-grade recovery rate. Methods The summary analyzed during 2003～2005 years through to strengthen encircles the stomach duodenal ulcer which the surgery time processing execution surgery treated to perforate the peritonitis and the purulent appendicitis and the peritonitis 587 examples, from 2003 January to 2004 June 282 examples is A group, mainly processed the measure to include: In front of the technique the preventive antibiotic use, the technique wild shop glue the surgery thin film, the careful surgery operate, the abdominal cavity accurate flushing, the idodine bend down the flushing margin. From July, 2004 to December, 2005 305 examples is B group.B group in presses in the foundation which A group processes to add with the technique you extreme flushes the abdominal cavity, may attract the recover of wire to suture measures continuously and so on peritoneum margin. Results Restores the aspect B group in the stomach and intestines function to surpass A group of (P<0.05) obviously. Margin a-grade recovery rate A group of 88.65%,B group 91.15%; Margin infection percentage A group of 4.97%,B group 3.93%.Conclusions Through strengthens encircles surgery time processing and the improvement surgery processes, has the promotion suppurative peritonitis patient stomach and intestines function to restore, enhances the margin a-grade recovery rate the function, has the practical value.
