高血压病QT离散度与左心室肥大和猝死的关系研究分析

摘　　要：目的探讨高血压病患者QT离散度(QTd)的变化与左心室肥大和猝死的关系。方法132例高血压病患者根据M型超声心动图检查分为2组,Ⅰ组69例患者有左心室肥大,Ⅱ组63例患者无左心室肥大。另外Ⅲ组为50位正常同龄人,作为对照组。测量并比较3组QTd的变化,同时比较Ⅰ组和Ⅱ组猝死的人数。结果Ⅰ组QTd为(68.54±31.44)ms、QTcd为(73.31±23)ms,Ⅱ组QTd为(43.42±21.25)ms、QTcd为(47.50±21)ms,2组相比有显著性差异,P<0.01。Ⅲ组QTd为(40.63±12.35)ms、QTcd为(43.82±25)ms,与Ⅱ组相比无显著性差异,P>0.05。Ⅰ组发生心脏性猝死者13例,Ⅱ组4例,有显著性差异,P<0.05。结论高血压病患者有左心室肥大者QTd明显增加,心脏性猝死者多。Objective To investigate the relation of left ventricular hypertrophy(LVH),sudden cardiac death and QT dispersion in patients with systematic hypertension.Methods 132 patients with systematic hypertension were divided into two groups according to M-mode echocardiogram examination.Group 1 included 69patients who met echocardio graphic criteria for LVH,and group 2 of 63patients did not have LVH.The group 3included 50 normal subjects as the control group.QT dispersion of 12-lead surface ECG was measured and compared in these groups.The number of sudden cardiac death in group 1 was compared with that in group 2.Results The QT dispersions and Qte dispersions were(68.54±31.44)ms and(71.31±23)ms respectively in group 1 of 69 patients with LVH,compared with those in group 2 of 63 patients without LVH(43.42±21.25)ms and(47.50±21)ms respectively,all P<0.01.The QT dispersions and QTc dispersions were(40.63±12.35)ms and(43.82±25)ms respectively in group 3 of 50 normal subjects,normal subjects,compared with those in group 2 of 63 patients without LVH(43.42±12.25)ms and(47.50±21)ms respectively.The differences were not significant,P>0.05.The number of sudden cardiac death was thirteen in group 1 and four in group 2,there was a significant difference,P<0.05.Conclusions The systematic hypertensive patients with LVH show an increase in QT dispersion and Sudden cardiac death.
