焦虑症和抑郁症患者认知电位P_(300)的比较

摘　　要：目的探讨焦虑症(AD)和抑郁症(CD)在事件相关电位(ERP)P_(300)检测中的不同表现。方法收集37例 AD 患者和32例 CD 患者以及36例健康成人对照组(NC)。使用美国仪器以及"听觉靶-非靶刺激序列"为诱发事件,完成 P_(300)检测。结果(1)AD 组、CD 组和 NC 组在靶潜伏期 Pz 脑区 P_3以及在靶波幅 Pz 脑区 P_2、P_3和非靶波幅 Pz 脑区 P_2上均有显著差异(P<0.01)。(2)AD 主成分 P_3表现为延迟,与 NC 组和 CD 组有极显著性差异(P<0.01)。(3)AD 组和 CD 组靶波幅 P_3和非靶波幅 P_2均见降低,与 NC 组比较也有显著性差异(P<0.05～<0.01)。结论提示作为反映 AD 和 CD 认知功能障碍的客观生理指标,P_(300)有可能作为 AD 和 CD 辅助诊断的一个脑电生理学标志。P_(300)检查可作为成人精神科的必查项目。Objective To investigate the changes of event related potentials P_(300)in patients with anxiety disorder (AD)and depression.Methods An ‘oddball’paradigm was used to elicit the auditory P_(300) from 37 patients diag- nosed as having AD according to CCMD-3,32 patients with depression and 36 normal controls by using a Nicolet Bra- vo Instrument.Results There was a significant difference of ERP-P_(300) among 3 groups in both latencies(Pz/P_3) and amplitudes(Pz/P_3,Pz/P_2)(P<0.01).Compared with normal control and AD groups,the depression group showed prolonged latency(P<0.01)and decreased amplitude(P<0.01).Compared with normal control groups, AD and depression patients showed decreased P_3 amplitude from target and P_2 amplitude from nontarget(P<0.05 or <0.01).Conclusion Auditory P_(300) ERP could served as a physiological index in the reflection of the cognitive disorder in AD and depression.P_(300) examination may serve as a necessary item for adult psychiatry.
