摘  要:目的 通过与头颅CT进行对照,评价近红外光密度仪诊断脑外伤颅内出血的可靠性.方法 脑外伤病人50例,年龄14~90(37.4±14.53)岁,男42例,女8例;均为从事故现场直接送至北京天坛医院,首先利用近红外光密度仪分别探查患者双侧额、颞、枕部位,记录并计算双侧对应部位的吸光度及其差值,以双侧差值0.45作为有无颅内出血的指标,然后进行CT检查,比较近红外探光密度仪诊断颅内出血的敏感性、特异性和可靠性.结果 50例脑外伤病人中近红外光检查阳性者27例,其中23例经CT证实,4例假阳性,23例CT检查无颅内出血病人中,20例经近红外光检查亦为阴性.近红外光学检查对脑出血诊断的敏感性为88.5%,特异性83.3%,可靠性86%.结论 近红外光谱对诊断外伤所致脑出血的结果可靠,无创,而且设备体积小,使用不受场地环境限制,适用于院前急救.Objective To evaluate the reliability of near-infrared spectroscopy (NIS) for diagnosing encephalic-hemorrhage in traumatic patients by comparing with CT examination. Method 50 traumatic patients, 42 male and 8 female aged 14-90(37.4±14.53) , were included in the study. All patients were sent to the emergency department directly from the incident regions. The near-infrared spectroscopy was firstly taken to the patients on both forehead, temporal and occipital positions. Read OD from the detector and calculate the difference in both parallel positions, take 0.45 of OD difference( Δ OD)as the criteria for diagnosing encephalic-hemorrhage, then performing CT examination. Result In 50 patients, 27 were diagnosed encephalic-hemorrhage by spectroscopy, among them 23 were proven by CT, in 23 normal CT cases, 20 were negative detected by spectroscopy. The sensitivity of near-infrared in diagnosing encephalic-hemorrhage was 88.5%, specificity was 83.3% and reliability was 86%. Conclusion It is reliable that near-infrared spectroscopy in diagnosing traumatic encephalic-hemorrhage for its high sensitivity, specificity and noninvasive. besides, the cubage of the device is small and its operation is not easily effected by circumstances. So it is suitable for using in pre-hospital emergency condition.
