应用系统论的方法学研究经颅多普勒超声的诊断与鉴别诊断

摘　　要：目的应用系统论及其方法研究和规范经颅多普勒超声(TCD)在脑血管狭窄、动静脉畸形、颈动脉海绵窦瘘等疾病中的诊断。方法应用系统论的方法学,结合文献资料和临床实例,以Willis环血流动力学在脑血管狭窄、动静脉畸形、颈动脉海绵窦瘘等病理状态下的特征形式作为系统集,Vs、Vd、Vm、PI、PTI、Vmca/Vica和SI为相关联的元素,探讨TCD的诊断要点。结果脑血管狭窄、动静脉畸形、颈动脉海绵窦瘘等分别代表Willis环的特殊病理状态下的系统集,以上关联的元素可组成具有不同系统特征的集-TCD诊断要点。结论Vs、Vd、Vm、PI、PTI、Vmca/Vica和SI等相关联的元素,在不同病理状态下的Willis环可构成相应的系统集的特征,进一步证明TCD作为一种无创性的超声诊断的可靠性和准确性。Objective To study and standardize TCD in the diagnosis of cerebral vascular stenosis,vascular spasm, arterio-venous deformity and cavernous sinus fistula of carotid artery using the systematism and its method.Methods In combi- nation with the literature and clinical cases,taking the characteristic form of hemodynamics at Willis cycle under the above patho- logic statuses as systemic sets,and the Vs,Vd,Vm,PI,PTI and Vmca/Vica as correlated elements,the main diagnostic points were investigated.Results The above pathologic statuses separately represented systemic sets under these conditions at Willis cycle,and from above correlated elements,the sets which owned the features of different systems-the main diagnostic points of TCD,was constituted.Conclusion The correlated elements of Vs,Vd,Vm,PI,PTI and Vmca/Vica at Willis cy- cle,under the above diseases,can constitute the corresponding features of systemic sets,and further to demonstrate the reliabili- ty and accuracy of TCD as a non-traumatic ultrasonographic diagnostic method.
