摘  要:目的探讨肠内营养支持对急性脑卒中患者营养状况和感染性并发症及近期神经功能恢复的影响。方法将76例脑卒中伴吞咽障碍的患者随机分为研究组(n=38)和对照组(n=38例),研究组患者于发病后3 d内给予鼻饲肠内营养,对照组患者于发病10 d后给予鼻饲肠内营养。观察两组患者入院21 d的血清白蛋白、前白蛋白和血红蛋白水平及神经功能缺损评分和感染率,随访60 d Barthel指数。结果入院21 d,研究组患者的血清白蛋白、前白蛋白及血红蛋白水平明显高于对照组;神经功能缺损恢复程度优于对照组;感染率低于对照组;随访60 d,两组患者的Barthel指数间差别无显著性意义,但研究组总分高于对照组。结论脑卒中伴吞咽障碍患者早期给予肠内营养支持可改善机体的营养状况,减少并发症,提高近期神经功能,改善预后。Objective To discuss the effects of early enteral nutritional support on the nutrition status,infective com- plications and the neurological deficit rehabilitation after acute stroke.Methods Total 76 patients with dysphagia were random- ized into experimental group and control group and received enteral nutritional.Experimental group included 38 patients who re- ceived nasogastric tube within 72h after admission,and others in control group received nasogastric feeding after more than 10 d. We observed the levels of serum albumin,prealbumin and haemoglobin at 21 d and Barthel index at 60 d in both groups,the neurological deficit rehabilitation and infective complications of the two groups were also compared.Results After 21 d,the levels of serum albumin,prealbumin and haemoglobin in experimental group were significantly higher than those in control group; the incidence of infective complications in experimental group was significantly lower than in control group;but the Barthel index at 60 d was not statistically different between the two groups. Conclusions Early nutritional support is helpful for increasing the nutrition status and the clinical outcome and reducing the frequency of infective complication in stroke patients with dysphagia.
