摘 要:目的通过观察卡哈尔间质细胞在正常对照组及先天性巨结肠患儿不同节段肠壁中的分布情况,进一步探讨先天性巨结肠症的发病机制。方法对24例先天性巨结肠患儿狭窄段及23例扩张段标本行c-kit免疫组化染色。以扩张段标本作为自身对照。另取5倒手术切除结肠标本作为正常对照。光镜观察卡哈尔间质细胞在正常对照组及巨结肠扩张段、狭窄段肠壁中的分布情况,计数阳性细胞数,分别对巨结肠扩张段与狭窄段、正常对照组与巨结肠狭窄段阳性细胞数目进行配对资料t 检验。结果先天性巨结肠狭窄段卡哈尔间质细胞数目比扩张段减少(P<0.01),比正常对照组亦有所减少(P<0.01)。且与扩张段和正常对照组相比,狭窄段卡哈尔间质细胞彼此间形成的网络状结构消失。结论先天性巨结肠症的发病与卡哈尔间质细胞的减少有密切关系。Objective The current study investigates the role of ICC in the pathogenesis of HD through observing the distribution of the cells in normal colon and HD' s different segments. Methods 24 aganglionic segments and 23 dilated segments of colon were collected. Each sections was stained by c - kit immunohistochemistry approach. The dilated segments were served as internal positive controls. Another 5 colon specimens were selected as controls. The distributions of ICC in between the muscle layers of bowel were studied using microscope. T test was applied with the numbers of positive cells of between dilated segment and aganglionic segment,normal colon and aganglionic segment. Results The diffrence in the quantity of ICC between the dilated segment and the agan- glionic segment was sutistically significant (P <0. 01), So does the diffrence in the quantity of ICC between the normal colon and the aganglionic segment(P<0.01). Conclusion Our study suggest that the pathogenesis of HD be relation to the abnormal distribution of ICC.
