乌拉地尔与硝普钠对充血性心力衰竭并早期肾功能不全患者的疗效对比观察

摘　　要：目的对比观察乌拉地尔与硝普钠对充血性心力衰竭(CHF)并早期肾功能不全患者的疗效。方法53例CHF 心功能Ⅲ-Ⅳ级并早期肾功能不全患者随机分成两组。A组27例为乌拉地尔组,B组26倒为硝普钠组,在常规治疗基础上,两组分别连续静脉泵入乌拉地尔2-12ug/(kg·min)或硝普钠0.5-2ug/(kg·min),5-7天。观察用药前后的心率(HR)、血压(BP)、左室射血分数(EF),肌酐(Cr)、尿素氮(BUN)的变化。结果纠正心衰A组总有效率96.2%,,B组总有效率100%,两组间疗效相比无显著性差异(P>0.05);用药后第24小时A组较B组心率下降明显(P<0.01),但治疗5-7天后两组心率、血压均无显著性差异(P>0.05);治疗5-7天后两组EF均较基线明显升高(P<0.01),但组间比较无显著性差异(P>0.05);两组治疗后较治疗前BUN、Cr均有下降趋势,但仅A组存在显著性差异(P<0.01)而B组无显著性差异(P>0.05),治疗后组间比较A组较B组BUN、Cr明显降低(P<0.05)。结论微量泵持续泵入乌拉地尔或硝普钠治疗cHF疗效均显著且作用平稳;乌拉地尔对肾功能改善作用优于硝普钠,而且乌拉地尔副作用小,更安全。Objective To contrast the efficacy of Urapidil and Sodium Nitroprusside in treating congest heart failure with early renal failure. Methods 53 patients with congest heart failure were admitted. We divided them into two groups randomly. A group has 27 patients who were adviced to last-inject Urapidil through vein in 5 -7 days 2 - 12ug/(kg·min). B group has 26 patients who were adviced to last - inject Sodium Nitroprusside through vein in 5 - 7 days 0. 5 - 2ug/( kg·min) . To observed, the heart rate (HR), blood pressure (BP) ,ejection fraction (EF) and examine Cr and BUN. Results The total efficiency of Urapidil and Sodium Nitroprusside is 96. 2% and 100% respectively(p < 0. 05). The HR - descend of A group is higher than B group after 24 hours(P < 0. 01) ,but the variance of HR and BP in two groups had not shown statistical notability after 5 -7days( P >0.05). The EF of both groups were higher than base - line after 5-7 days respectively( P <0.01) , but the variance of EF between two groups had not shown statisti- cal notability (P > 0. 05). The Cr and BUN were lower than base - line in both groups after 5 - 7 days, but the statistical notability only had shown in Urapidil group( P <0.01). The variance of Cr and BUN had shown statistical notability between two groups after S -7days (p < 0.05). Conclusion Both Urapidil and Sodium Nitroprusside is efficacy and safety in treating congest heart failure. Urapidil is excelled to Sodium Nitroprusside in reducing Cr and BUN. The side - effect of Urapidil is mild. Urapidil is efficacy and safty in treating congest heart failure with early renal failure.
