摘 要:目的探讨应用氢气呼气试验检测乳糖吸收不良,蔗糖吸收不良,小肠细菌过度生长,胃排空障碍及小肠传递时间。方法采用不同底物试餐测定空腹和试餐后呼气氢浓度动态变化,判断消化道功能状态。结果95例成人乳糖吸收不良发生率47.4%,其中18-35岁、35-60岁分别为40.4%、57.9%,(P>0.05);34例3-7岁儿童乳糖吸收不良发生率67.6%,成人组与儿童组比较(P<0.05)。68例成人蔗糖吸收不良发生率8.82%,其中18-35岁、35-60岁分别为8.51%、9.52%,(P>0.05)。59例成人小肠细菌过度生长检出率为18.6%,对11阳性者经抗生素治疗,试餐后呼气氢浓度较治疗前均显著下降(P<0.01)。110例成人胃排空障碍检出率14.5%,对16例阳性者经吗叮啉治疗,空腹呼气氢浓度较治疗前显著下降(P<0.01)。34例成人平均小肠传递时间为106.76±22.39min,其中18-29岁、29-60岁分别为100.50±21.27min、115.70±21.56min(P=0.05)。结论消化道碳水化合物酶活性、细菌的定植状态、胃排空能力、肠蠕动能力均与呼气氢浓度有关。乳糖酶活性儿童低于成人。呼气氢试验简单,无创,安全,有广泛临床应用价值。Objective To determine hydrogen breath test detecting lactose malabsorption,saccharu malabsorption,small intestine bacteria over - growth, gastric emptying dysfunction,small intestine convection time. Methods Judge digestive tract function by examining expiratory hydrogen concentration change during empty stomach and after eating different substrate.Results Lactose malabsorption occurred in 47.4% of 95 adult subjects, 40.4% at 18 -35year,and 57.9% at 35 -60year, respectively,but in 34 3 -7year children subjects the ratio was 67. 6%. The comparison between adult group and children group was P <0. 05. Saccharase malabsorption occurred in8. 82% of 68 adult cases, 8. 51% at 18 -35year,and 9. 52% at 35 -60year,respectively.Small intestine bacteria over -growth occurred in 18.6% of 59 adult subjects, 11 positive cases were treated by antibiotic,compared with untreatment, expiratory hydrogen concentration drop significantly after eating (P<0.05).The ratio of gastric emptying dysfunction in 110 cases was 14. 5% , among 16 positive subjects domperidone was taked orally, FBH concentration descended remarkablely(P<0.05). In 34 adult subjects small intestine convection average time was 106. 76±22. 39 minutes, 100. 50####21. 27 at 18 - 29year, and 115. 70±21. 56 at 29-60year. Conclusion Carbohydrate digestive enzyme activity,bacterial growth, gastric emptying function and vermiculation ability related to expiratory hydrogen concentration. Children lactase activity was deficient compared with adult.Hydrogen breath test was easy did, no wound, and safety, so which will be widespreadly applied.
