血液肿瘤儿童治疗中的行为问题及影响因素比较

摘　　要：目的探讨血液肿瘤儿童在治疗中的行为问题及家长对其行为的影响。方法对 57例血液肿瘤患儿进行 Achenbach儿童行为量表( CBCL)及家长 90项症状自评量表( SCL- 90)测定,设 62例同期住院儿童及 31位家长为对照组。结果血液肿瘤儿童组行为问题总检出率为 21.1%,男孩在交往不良、躯体诉述、违纪,女孩在抑郁、社会退缩、躯体诉述因子与对照组有显著差异。家长 SCL- 90各因子分均高于对照组,两组比较差异有极显著差异( P< 0.01)。其敌对因子、人际关系、偏执、焦虑、强迫等因子与患儿的分裂样、抑郁、体诉、社会退缩、多动、攻击、违纪呈正相关。结论血液肿瘤儿童在治疗中存在明显的行为问题,与治疗过程、家长心理状况因素有关。Objective We are concentrated in investigation the behavior problems and the influence on their behavior from their parents during the treatment of children with hematological tumor.Method We investigated 57 children with hematological tumor in hospital (tumor group) using CBCL(Child Behavior Checklist),and their parents using SCL－ 90(Symptom Checklist－ 90).62 nonhematological tumor cases in hospital and 31 parents (controlled group)were studied as contrast at the same time.Result The total detectable rate is 31.1％ in the behavior problems of children with hematological tumor.There are such significant differences between the two groups of hematological tumor and the contrast as the boys's social problems,somatic complaints,and delinquent behavior,and the girls' depression,social withdrawal and somatic complaints.The SCL－ 90 score of parents are higher than those of the contrast with remarkable difference (P< 0.01),in which the parents' hostility,relationship,bigotry,anxiety,compulsion being in positive correlation with the children's divisional,depression,somatic complaints,social withdrawal,hyperactivity,aggression and delinquent behavior.Conclusion There are obvious behavior problems during the treatment of children with hematological tumor,which are in correlation with the therapeutic process and the parents' psychology state.
