摘  要:目的 建立端侧吻合修复臂丛神经损伤的实验模型。方法 选用体重为 180g~ 2 0 0g的Wister大白鼠 36只 ,随机分为三组 ,每组 12只 ,每只右侧神经外膜开窗 (0 2cm× 0 2cm) ,左侧外膜未开窗。将臂丛后束损伤后的桡神经远断端端侧吻合于外膜开窗的内侧束远端 0 5cm部位 ,分别于术后 1月、 2月、 3月在麻醉下行电生理、肌纤维截面积、肌肉湿重的检测。术后 3月同时行内侧束吻合口远、近端神经纤维数目及神经束截面积测定。结果 桡神经端侧吻合后 ,随时间的延长 ,运动神经诱发电位潜伏期缩短 ,波幅增高 ,传导速度加快 ,肱三头肌肌湿重及肌纤维截面积值逐渐加大 ,但开窗组优于未开窗组 (P <0 0 5 )。端侧吻合对主干神经无影响。结论 后束损伤的桡神经端侧吻合于外膜开窗的内侧束的模型简便易行、可靠 ,有较好的实用价值。Objective To establish a r at model to repair brachial plexus injuries by end-to-side neurorrhaphy.Methods Thi rty-six rats were randomly divided into three groups,with 12 rats each.The right radial nerve were repaired with end-to- side fashion,the distal end being suture d to the epineurial window(0 2cm×0 2cm)0 5cm distal of the medial fasciculus.Ele ctrophysidogical,cross sectional area of the muscle fibers and muscle wet weight were detected 1,2,3months postopera tion respectively.The number and cross s ectional area of nerve fibers of the med ial fasciculus were measured 3 months af ter op eration.Results Both window and without window of end-to-side attachment were able to allow nerve regeneration,but the rege neration quility of the former made sign ifcant different(P<0 05)to the latter.Th ere was no influence on the normal nerve trunk.Conclusion This model suc ceeded i n its relatively simple and reliable,can be provided a direct way for experimental s tudy of human brachial plexus injuries r epaired with end-to-side technique.
