摘  要:目的 检测口腔鳞癌患者化疗前后P-糖蛋白(P-gp)、肺耐药相关蛋白(LRP)、多药耐药相关蛋白(MRP)、拓朴异构酶Ⅱα(TopoⅡα)的蛋白质表达,分析其与临床病理参数及化疗敏感性的关系。方法 采用流式细胞术的方法,对34例口腔鳞癌患者术前诱导化疗前后P-gp,LRP,MRP,TopoⅡα的蛋白质表达进行测定。化疗方案为平阳霉素16mg/m2,第1-10天。结果 ①口腔鳞癌临床化疗有效率为20.6%,其中CR 1例、PR 6例、NC 26例、PD 1例。多因素分析结果表明P-gp的表达水平与口腔鳞癌对平阳霉素化疗的敏感性呈负相关(P=0.041),而LRP,MRP,TopoⅡα的蛋白质表达均与化疗反应无明显的相关性;同时单因素分析发现化疗有效者的P-gp表达水平明显低于无效者(P<0.05),而LRP、MRP及TopoⅡα蛋白质表达在二者并无显著差异;②在口腔鳞癌组织中,同时存在多种MDR相关蛋白超表达的现象,其中P-gp平均表达率为27.7%±30.2%,LRP为41.1%±17.9%,MRF为43.9%±16.1%;癌组织的MDR相关蛋白表达水平明显高于癌旁组织(P<0.05),同时,它们在化疗前后的表达水平并无显著差异,表明口腔鳞癌的多药耐药性与内在性耐药有关;③P-gp的表达水平与肿瘤细胞的分化程度呈正相关(P<0.05),分化越好,表达水平也越高(P<0.05),但与临床分期、淋巴结转移无关;而LRP、MRP?The aim of this study was to examine the expression of protein of multidrug resistance gene (MDR1), lung resistance - related protein (LRP), multidrug resistance - associated protein ( MRP), topoi-somerase Ⅱα(Topo Ⅱα) before and after preoperative chemotherapy in patients with oral squamous cell carcinoma (OSCC), and then to evaluate their roles in relation to clincopathologic factors and response to chemotherapy. Methods 34 cases with OSCC were given one cycle chemotherapy before operation. Chemotherapy regimen consisted of single Pingyangmycin agent. The expression of MDR - related protein were measured by flow cytometry. Results The clinical response rate in OSCC was 20.6% (CR 1 case, PR 6 cases, NC 26 cases, PD 1 case). Multivariate analysis showed that the high P- gp expression level was significantly associated with a poor response to chemotherapy ( P = 0.041) and no association was found between the protein expression of LRP, MRP, Topo Ⅱα and response to treatment. Moreover, univariate analysis show that the expression level of P - gp in responders was lower than that in non-responders ( P < 0.05) , and that the protein expression of other MDR - related markers were no significantly difference between the responders and non - responders. The mean expression levels of protein of MDR- related markers in OSCC were 27.7%±30.2% for P-gp, 41.1%±17.9% for LRP, 43.9%±16.1% for MRP, respectively. The expression levels of protein of MDR - related markers in cancer tissues were higher than those in tumor adjacent oral mucosa ( P < 0.05). No significant change of expression levels of protein of the MDR - related markers was observed between the samples taken before chemotherapy and after chemotherapy. Those results indicated the multidrug resistance of OSCC might be derived from the intrinsic multidrug resistance.
