43例原发性皮肤淋巴瘤临床分析

摘　　要：目的　探讨原发性皮肤淋巴瘤的临床特点。方法　回顾性分析我院 1984～ 1998年间收治的 4 3例原发性皮肤淋巴瘤的临床资料。结果　原发性皮肤淋巴瘤占我院同期收治淋巴瘤的0 96 %。治疗方法以化和 或放疗为主 ,部分患者手术切除原发灶。整组病例的总有效率为85 3% ,CR率 73 5 % ,5年总生存和无病生存率分别为 71 6 %和 6 2 5 %。蕈样真菌病 S啨ｚａｒｙ综合征 (mycosisfungoidesandS啨ｚａｒｙｓｙｎｄｒｏｍｅ ,MF SS)组的 5年总生存和无病生存率分别为 6 7 1%和5 2 9% ,非MF SS组为 75 4 %和 71 4 % ,14例非MF SS性T细胞淋巴瘤为 6 7 1%和 6 1 5 %。88 8%的患者首次复发部位以皮肤为主。结论　原发性皮肤淋巴瘤是少见的有独特生物学行为的结外淋巴瘤 ,对化放疗敏感 ,大多数亚型的原发性皮肤淋巴瘤预后好于相同类型的结性淋巴瘤。Objective To explore the clinical characteristics of primary cutaneous lymphoma.Methods The clinical records of 43 cases of primary cutaneous lymphoma treated in 1984 to 1998 were retrospectively reviewed.Results Primary cutaneous lymphoma accounts 0.96%of all non-Hodgkin's lymphomas presented in the corresponding period.Most patients received chemotherapy and/or irradiation.The total responding rate is 85.3%,with a complete response(CR)rate of 73.5%.The 5-year overall and disease free survival rate are 71.6%and 62.5%respectively.The 5-year overall and disease-free survival rate for patients with mycosis fungoides and Sézary syndrome(MF/SS),non-MF/SS or non-MF/SS T cell primary cutaneous lymphoma are 67.1%and 52.9%,75.4%and 71.4%,67.1%and 61.5%respectively.The first relapse mainly involved skin in 88.8%of patients.Conclusion Primary cutaneous lymphoma is a rare entity of extranodal lymphoma with unique biology and clinical characteristic.It is sensitive to irradiation and chemotherapy with a relatively good outcome.
