摘　　要：目的 :应用自体造血干细胞移植以及体外净化骨髓技术治疗恶性实体瘤 ,探索肿瘤治疗学的新方法。方法 :13例实体瘤患者自体骨髓移植 6例 ,自体外周血干细胞移植 7例 ,预处理方案为全身照射加大剂量化疗或大剂量化疗 ,体外净化采用Polyerga 10 0 μg L加IL 2 10 0 0u ml加入骨髓 ,37℃孵育 6h ,观察与评价疗效及治疗相关并发症。结果 :13例中完全缓解 (CR) 8例 ,部分缓解 (PR) 2例 ,总体治疗有效率 76 .9% ;移植相关死亡 2例 (15 .4% ) ,随访 3～ 2 6个月 ,现存活 8例。结论 :Polyerga加IL 2净化自体骨髓 ,可起到清除残留微小病变的作用 ;自体造血干细胞移植可提高实体瘤的缓解率 。Aim: To research the new directions in treatment for solid tumor with autologous hematopoietic stem cell transplantation (HSCT) and purging in vitro with polyerga and IL 2 . Methods: Of the 13 patients , 6 were in ABMT and 7 in PBSCT. Preparative regimens was composed of total body irradiation and intensive chemotherapy or intensive chemotherapy only. Polyerga 100 μg/L and IL 2 1 000 u/ml was mixed with bone marrow and hatched at 37 ℃ and in 5 % CO 2 for 6 hours. Results: In this study, 8 cases achieved CR (61.5%) and 2 cases, PR (15.4%). The overall response rate was 76.9%. Two patients died of bleeding or serious mycotic infection. Conclusion: Intensive chemotherapy with autologous hematopoietic stem cell transplantation can improve the CR rate and DFS of the patients with solid tumors. Purging in vitro with Polyerga and IL 2 can be conductive to elimination of MRD.
