血液病合并侵袭性真菌感染的治疗(附21例分析)

摘　　要：目的:探讨血液病患者合并侵袭真菌感染(IFI)应用氟康唑、伊曲康唑和脂质体两性霉素B治疗的结果分析。方法:选择交大一附院血液科2005-01～2007-07收治的血液病合并IFI患者21例,单用常规剂量的氟康唑7例、伊曲康唑7例、脂质体两性霉素B7例;氟康唑无效者改用伊曲康唑2例,改用脂质体两性霉素B2例。结果:21例IFI总的治愈率和有效率分别为28.57%和42.86%。在各诊断组间差异无显著性意义(P>0.05)。单药氟康唑治疗组的有效率显著低于伊曲康唑、脂质体两性霉素B治疗组,用氟康唑治疗无效改用伊曲康唑或脂质体两性霉素B治疗的有效率分别为100%和50%。结论:伊曲康唑和两性霉素B治疗IFI的疗效相似,明显高于氟康唑。Objective:To investigate the efficacy of fluconazole.Itraconazole and liposome amphotericin B theraphy IFI patients with hematopathy.Methods:Choice 21 IFI patients with hematopathy from January of 2005 to July of 2007 in JiaoDa First Hospital.Only routine dose fluconazole 7 cases.itraconazole 7 cases.liposome amphotericinB 7 cases.Fluconazole inefficacy change to itraconazole 2 cases.and liposome amphotericinB 2 cases.Results:Total cure rate was 28.57% and efficient rate was 42.86%.the different diagnose groups was not significant(P>0.05).The efficient rate of only fluconazole was obvious lowly itraconazole and liposome amphotericinB groups.The efficient rates of fluconazole inefficacy change to itraconazole or liposome amphotericinB was 100% and 50%.Conclusions:Therapeutic effect of itraconazole and liposome amphotericinB treatment IFI is analogous.It was greater in itraconazole and liposome amphotericinB groups than in fluconazole groups.
