膀胱浆细胞样尿路上皮癌临床病理特征分析(附4例报告)

摘　　要：目的探讨膀胱浆细胞样尿路上皮癌临床病理特征。方法采用常规HE和免疫组化染色方法,观察4例膀胱浆细胞样尿路上皮癌组织学形态,通过临床随访和文献复习进行讨论。结果浆细胞样尿路上皮癌肿瘤细胞具有浆细胞样特征,免疫组化CK和CK7阳性,淋巴细胞和浆细胞标记为阴性。患者通常无典型临床表现,确诊时多为晚期。结论浆细胞样尿路上皮癌进展快,预后差,早期诊断难。本病的病理诊断需要和其他浆细胞样病变鉴别。Objective To discuss the clinicopathological features of plasmacytoid urothelial carcinoma(PUC)of the bladder.Methods Four cases of PUC of the bladder were observed under lightmicroscope,HE and immunohistochemical staining.Pathological and clinical data were analyzed by comparison with early reported cases in the literature.Results The tumor cells were diffusely distributed and exhibited plasmacytoid profiles.Immunohistochemically,the tumor cells were positive for CK and CK7 but negtive for markers of plasma cells and lymphocytes.The confirmed diagnosis was frequently found at an advanced stage due to the absence of typical clinical presentation.Conclusion PUC of the bladder possesses quick progress,poor prognosis and difficulty of early diagnosis.Differentiation between the pathological diagnosis of PUC of the bladder and other plasmacytoid malignant lesion are necessary.
