两种不同形状骨膜下种植体愈合效果的对比研究

摘　　要：目的:对比两种不同形状骨膜下种植体(SI)的愈合结果,为颌骨SI的形状设计提供依据。方法:定制的格状、网状SI共24枚植入6只猪颅顶,每只两种SI各2只。4周后处死取材,测试SI的拉出力并计算出其与组织的结合强度。对SI及周围组织标本进行组织学检查。结果:18枚SI(9枚格状、9枚网状)正常愈合。网状SI平均拉出力大于格状SI 29%(P<0.05),但二者结合强度分别为0.34±0.06 MPa、0.32±0.1 MPa,无显著差异(P>0.05)。网状、格状SI各2枚被新生骨覆盖并发生骨结合;其余14枚SI均为纤维愈合,二者间无明显差异。结论:在植入范围相同的前提下,颌骨SI选择实体接触面积较大的网状SI会增加植体的组织结合力。Objective:To compare the tissue integration of the subperiosteal implants(SIs) with two different shapes,and to provide better design proposal for jaw bones SIs.Methods: Six pigs were submitted into this study.Four SIs(2 mesh-shaped and 2 grid-shaped) were fabricated and implanted on the calvarium of each pig.The pigs were sacrificed after 4 weeks,and the integration of implants was evaluated biomechanically using pull-out test and histologically by light microscope.Results: Nine mesh-shaped SIs and nine grid-shaped SIs healed normally.In each kind of SIs,2 SIs were osseointegration and 7 were fibrointegration.The average pull-out force of the mesh-shaped SI was higher than that of the grid-shaped SI by 29%(P0.05).Conclusion: With the same implant area,the mesh-shaped SIs will present a greater implant-tissue attachment force because of the less void in its shape.
