中心静脉导管胸腔闭式引流联合顺铂胸腔灌注对癌性胸水的治疗应用

摘　　要：目的:探讨中心静脉导管胸腔闭式引流联合顺铂胸腔灌注对癌性胸水的治疗作用。方法:我院2008年12月～2009年8月收治的癌性胸水患者49例随机分为观察组(25例)和对照组(24例),对照组采用常规胸腔穿刺抽胸水后注入顺铂治疗,观察组采用中心静脉导管胸腔闭式引流联合顺铂胸腔灌注治疗。结果:观察组的总有效率为88.0%,明显高于对照组的62.5%(P<0.05);观察组患者Kamofsky评分、NRS评分、QOL评分的改善情况均明显优于对照组(P<0.05);观察组的不良反应发生率为12.0%,略低于对照组的16.7%(P>0.05)。结论:中心静脉导管胸腔闭式引流联合顺铂胸腔灌注对癌性胸水疗效显著,且不良反应少,能够提高患者的生活质量。Objective:To investigate the therapeutic effects of closed thoracic drainage with central venous catheter combined with Cisplatin intrapleural infusion in the treatment of cancerous hydrothorax.Methods:From December 2008 to August 2009 in our hospital,49 patients with cancerous hydrothorax were randomly divided into 25 cases of observation group and 24 cases of control group,the control group used conventional pleural effusion and Cisplatin intrapleural infu sion,the observation group used closed thoracic drainage with central venous catheter combined with Cisplatin intrapleural infusion.Results:The total effective rate of observation group was 88.0%,it was significantly higher than 62.5% of control group (P0.05).Conclusion:Closed thoracic drainage with central venous catheter combined with Cisplatin intrapleural infusion in the treatment of cancerous hydrothorax has significant curative effect and fewer side effects,it can improve the quality of life of patients.
