益生菌在自发性细菌性腹膜炎中的应用

摘　　要：目的:探讨微生态制剂益生菌在预防肝硬化自发性细菌性腹膜炎中的临床效果。方法:随机选择广东省惠州市中心人民医院2008年1月～2009年12月收治的肝硬化自发性细菌性腹膜炎患者93例,随机分为两组,治疗组(抗生素联合益生菌治疗)52例,在常规抗生素治疗的基础上给予益生菌治疗,疗程14d;对照组(抗生素治疗)41例进行对比治疗,仅用常规抗生素治疗。分别观察治疗后即时疗效和追踪观察半年的远期效果。结果:治疗后即时查对照组肠球菌[(8.21±1.39)lgCFUg-1)]较治疗前[(8.63±1.51)lgCFUg-1]减少(P<0.05),双歧杆菌[(10.58±1.46)lgCFUg-1]较治疗前[(9.81±1.13)lgCFUg-1]增加(P<0.05);随访3个月,对照组有18例再发腹膜炎,再发率为43.9%(18/41),而治疗组有16例,再发率为30.8%(16/52),治疗组再发率低于对照组(P<0.05);9例(治疗组5例,对照组4例)患者出现肝功能衰竭、上消化道大出血、肝性脑病等原因死亡,病死率分别为对照组9.8%(4/41),治疗组9.6%(5/52),差异无统计学意义(P>0.05)。结论:抗生素联合益生菌治疗肝硬化自发性细菌性腹膜炎,可有效提高疾病的近期治疗效果,并能在一定程度上预防腹膜炎的发生,值得在临床中推广应用。Objective:To explore the clinical effects of probiotics on primary prevention of peritonitis in liver cirrhosis.Methods:During January 2008-December 2009,the People’s Hospital of Huizhou City admitted 93 cases of liver cirrhosis patients with primary peritonitis were randomly divided into two groups,treatment group (antibiotics combined with pro biotic treatment),52 cases,probiotic treatment on the basis of conventional antibiotics,given treatment regimen 14 d;con trol group (antibiotic treatment) 41 cases,only conventional antibiotics.Effects were observed immediately after treatment and followed for three months long-term results.Results:The control group after treatment,enterococci immediately was checked [(8.21±1.39) lgCFUg-1] less than before treatment [(8.63 ±1.51) lgCFUg-1] (P0.05).Conclusion:Antibiotic treatment of liver cirrhosis primary peritonitis combined with probiotics can effectively improve short-term treatment of diseases,and to certain extent,prevent the occurrence of peritonitis,it is expected to be widely ap plied in clinic.
