全脑放疗联合后程三维适形放疗治疗脑转移瘤的临床研究

摘　　要：[目的]观察全脑放疗(WBRT)联合后程三维适形放疗(3D-CRT)治疗脑转移瘤的疗效及不良反应。[方法]56例患者随机分为全脑放疗组(WBRT/全脑组)26例和全脑放疗联合后程适形放疗组(WBRT+3D-CRT/联合组)30例。两组均行全脑放疗30Gy/10次/2周,联合组缩野对转移灶行3D-CRT推量照射15Gy/5次/1周,总量45Gy;观察两组疗效及不良反应。[结果]联合组完全缓解(CR)、部分缓解(PR)分别为40.0%和50.0%,而全脑组分别是15.4%和46.2%,联合组有效率(CR+PR)明显高于全脑组(P<0.05);联合组和全脑组1、2年生存率分别是53.3%、26.6%和26.6%、7.7%(P<0.05);无明显毒副反应发生。[结论]全脑放疗联合后程3D-CRT治疗脑转移瘤能明显提高局部控制及近期疗效,1年及2年生存率均提高,无明显不良反应。[Objective] This clinical study was to evaluate the efficacy of WBRT combined with three-dimensional conformal radiationtherapy on brain metastases(BM),and investigate side effect.[Methods] A total 56 patients with BM with 1 to 3 intracranial lesions were treated using WBRT first,the WBRT doseage was 30Gy(10f/2W).Then 30 patients among them were treated using 3D-CRT after WBRT.The 3D-CRT dose was 15Gy in 5 fractions.[Results]The median survival time was 9.2 months vs 13.7 months in WBRT group vs WBRT combining 3D-CRT group.The 1-and 2-year OS rates were respectively 26.6% vs 53.3% and 7.7% vs 26.6%.The 1-year and 2-year OS rates were significantly higher in the patents with 3D-CRT combination WBRT treatment.[Conclusions]The three-dimensional conformal radiotherapy combined with whole brain irradiation for brain metastases could control the focus lesions of brain effectively,prolong survival time and improve life quality as well.It has few side effects.
