胰腺实性假乳头状瘤的CT诊断

摘　　要：目的探讨胰腺实性假乳头状瘤的MDCT特征。方法回顾性分析经手术病理证实的20例胰腺实性假乳头状瘤的CT表现及其临床资料。结果 20例患者中,女性占18例,30岁以下有17例。肿瘤有包膜、边界清楚,呈圆形、卵圆形或分叶状,多数体积较大,位于胰头部或尾部,5.0cm以上有18例。据囊实性成分的多少分为3型:Ⅰ型实性;Ⅱ型囊实性;Ⅲ型囊性,其中囊实性最多见,实性部分均有不同程度的强化,实性部分或包膜钙化者有4例。结论胰腺实性假乳头状瘤CT表现具有一定的特征性,结合青年女性好发的临床特点,术前可作出准确诊断。Objective To discuss the MDCT of solid pseudopapillary tumor of the pancreas(SPTP).Methods The CT and clinical findings of 20 cases with surgery and pathology proved SPTP were retrospectively analyzed.Results Among the 20 cases of SPTP,18 cases occurred in female and 17 cases under 30 years-old.The masses with peritumoral capsules appeared round,oval or lobulate.Most of them were of large size and located at the head and the tail of pancreas.18 tumors were larger than 5.0 cm.The tumors were divided into three types according to the density:Ⅰtype solid;Ⅱtype solid-cystic;Ⅲ type cystic,most of them were solid-cystic,and the solid components showed different degree of enhancement.The calfications of the solid components and the capsules were identified in 4 cases.Conclusions The CT characteristic findings of SPTP might be of great value for the diagnosis.The SPTP can be diagnosed accurately combining CT characteristics with the clinical feature when it occurrs in young female.
