缺血性白质脑病患者认知障碍和情感障碍事件相关电位研究

摘　　要：目的探讨缺血性白质脑病患者认知障碍、情感障碍的特点及规律,以及事件相关电位研究。方法收集缺血性白质脑病患者480例和正常对照86例,分别应用缺血量表、MMSE和Hamilton焦虑量表及24项版本抑郁量表、生存质量量表进行测评,并进行事件相关电位P300、N400检测。结果 480例缺血性白质脑病中,LA1级118例(24.58%),LA2级153例(31.88%),LA3级134例(27.92%),LA4级75例(15.63%)。脑白质损害不同分级之认知障碍、情感障碍比较有统计学差异,随着脑白质损害加重、分级增高,患者生存质量评分明显降低,认知障碍和情感障碍加重,事件相关电位P300和N400潜伏期明显延长,波幅明显降低。结论缺血性白质脑病可造成患者严重认知、情感障碍,事件相关电位P300、N400可作为早期诊断缺血性白质脑病认知障碍有效工具之一。Objective The characteristics and rules of cognitive impairment and the emotional disorder of the patients with ischemic white matter encephalopathy were researched in this article.The event related potential was also discussed.Methods 480 patients with the ischemic white matter encephalopathy and 56 normal people were assessed by Hachinski Ischemic Score,MMSE,Hamilton anxiety scale,24-Hamilton-depression scale,stroke-specific quality of life and the event related potential P300,N400,respectively.Results In 480 patients of white matter encephalopathy,118(24.58%) were LA1,153(31.88%) LA2,134(27.92%) LA3,75(15.63%) LA4.There were significantly statistic difference in different levels of the cognitive impairment and emotional disorders.With the damage and grade of white matter increasing,the cognitive impairment and emotional disorders became worse,the event related potential P300,N400 significantly prolonged,and the amplitude decreased obviously.Conclusions Ischemic white matter encephalopathy would cause cognitive impairment and emotional disorder.The event related potential P300,N400 could be one of the effective tools which can diagnose ischemic white matter encephalopathy.
