摘 要:目的研究乙型肝炎病毒(HBV)基因分型与前C基因及基本核心启动子(BCP)突变,为临床优化乙型肝炎治疗方案提供决策依据。方法征集98例未接受过抗病毒治疗的慢性乙型肝炎(CHB)患者为研究对象。通过乙肝表面抗原(HBsAg)基因测序进行基因分型。同时对前C基因及BCP进行测序分析。结果研究对象中HBV基因分型为:A:11例(11.2%);B:20例(20.4%);C:56例(57.1%);D:8例(8.2%);B/C混合型:3例(3.1%)。与其他基因型相比较,基因B和C型的患病率较高。基因型C的前C基因突变率为46.9%(46/98),较其他基因型的高,差异具有统计学意义(χ2=47.85,P<0.01);基因型C的BCP突变率为41.8%(41/98),较其他基因型的高,差异具有统计学意义(χ2=67.14,P<0.01)。结论 HBV基因分型以B、C基因为主。基因型C与BCP和前C基因突变密切相关。Objective To explore the relationship between prevalence of precore and basal core promotor(BCP) mutants and hepatitis B virus(HBV) genotyping,and it may provides the laboratory informations for a physician to planning clinical therapy for patients.Methods Ninety-eight chronic hepatitis B(CHB) patients who were antivirus-untreated were recruited in this study.HBV genotyping were performed by gene sequencing.Meantime,precore and BCP of HBV were also sequenced in all patients.Results HBV genotypes A,B,C,D and B/C mixed genotype were found by gene sequencing in this study,their proportions were 11.2%(11/98),20.4%(20/98),57.1%(56/98),8.2%(8/98) and 3.1%(3/98) respectively.There were higher morbidities of genotypes B and C than that of the others.There were higher frequence of mutations of precore(46.9%,46/98) in genotype C than that of the others,there was a significant difference in the mutations of precore gene with genotypes(χ2=47.85,P<0.01).Frequence of mutations in BCP was higher in genotype C(46.9%,46/98) than that in the others,there was a significant difference in the mutations of BCP with genotypes(χ2=67.14,P<0.01).Conclusions Genotypes B and C are the most prevalent HBV genotypes in this study,and genotypes C was closely related to the presence of BCP and precore mutants.
