肺癌患者血清中肿瘤标志物检测的临床意义

摘　　要：目的探讨血清肿瘤标志物癌胚抗原(CEA)、胃泌素释放肽前体(pro-GRP)、神经特异性烯醇化酶(NSE)、鳞状上皮细胞癌抗原(SCC-Ag)、细胞角蛋白19片段(CYFRA21-1)对肺癌辅助诊断以及疗效评价的临床意义。方法应用ELISA法检测50例健康人、60例肺部良性疾病患者和92例肺癌患者化疗前后血清中5种肿瘤标志物水平。结果肺癌患者血清中肿瘤标志物水平明显高于健康组和良性肺部疾病组(P<0.05);CEA在腺癌中的检测水平明显高于其他类型肺癌(P<0.01);NSE、pro-GRP在小细胞性肺癌(SCLC)中的检测水平明显高于其他类型肺癌(P<0.01);SCC-Ag在鳞癌中的检测水平明显高于其他类型肺癌(P<0.05);CYFRA21-1在鳞癌中的检测水平明显高于其他类型肺癌(P<0.01);化疗后肺癌患者5种血清肿瘤标志物水平明显低于化疗前水平。结论肿瘤标志物CEA、pro-GRP、NSE、SCC-Ag、CYFRA21-1在肺癌的辅助诊断、鉴别诊断以及疗效评价中有很重要的临床意义。Objective To investigate the clinical significance of carcinoembryonic antigen(CEA),precursor of gastrin-releasing peptide(pro-GRP),neurone specific enolase(NSE),squamous cell carcinoma antigen(SCC-Ag),cytokeratin-19-fragments(CYFRA21-1)in diagnosis and response monitoring in patients with lung cancer.Methods The levels of five serum tumor markers were detected by ELISA in 50 healthy adults,60 patients with lung benign disease and 92 patients with lung cancer before treatment and after chemotherapy.Results The levels of the five serum tumor markers in patients with lung cancer were significantly higher than that in patients with lung benign disease and healthy adults(P<0.05).The level of CEA in patients with adenocarcinoma was significantly higher than that of other subtypes of lung cancer(P<0.01).The levels of pro-GRP and NSE in patients with small cell lung cancer(SCLC) were significantly higher than those of other subtypes(P<0.01).The level of SCC-Ag in patients with squamous-cell carcinoma was significantly higher than that of other subtypes(P<0.05).The level of CYFRA21-1 in patients with squamous-cell carcinoma was significantly higher than that of other subtypes(P<0.01).The levels of the five serum tumor markers in patients with lung cancer after treatment were significantly lower than that of patients before treatment.Conclusions The five tumor markers are valuable to diagnosis,differential diagnosis and response monitoring for patients with lung cancer.
