缺氧相关蛋白碳酸酐酶-9在乳腺癌的表达及其生物学意义

摘  要:目的评价缺氧相关蛋白碳酸酐酶-9(CA-9)在乳腺癌中的表达状况及与临床病理指标的可能关系,探讨缺氧在乳腺癌的发生和发展中的作用。方法采用免疫组织化学方法检测100例乳腺浸润性导管癌CA-9的表达,并评价其与病理分级、临床分期、转移以及预后之间的关系。结果 100例乳腺癌组织CA-9表达阳性29例,阴性71例。CA-9的表达与乳腺癌淋巴结转移(P=0.015)、临床分期(P=0.018)以及预后(P=0.0001)密切相关;与年龄(P=0.375)、肿瘤大小(P=0.288)和肿瘤病理分级(P=0.526)无关。CA-9的表达与乳腺癌患者的总体生存率(Log-rank=15.638;P=0.0001)和无瘤生存率(Log-rank=14.678;P=0.0001)有关。多因素分析结果提示CA-9是乳腺癌独立的预后因子(P=0.002)。结论 CA-9的表达在乳腺癌中有统计学意义,其表达随转移和临床分期升高而增强,提示缺氧微环境对乳腺癌的发生及侵袭、转移起着重要作用。缺氧标志物CA-9可能是乳腺癌的预后因素之一。Objective Invasive breast ductal carcinoma is characterized by a heterogeneously hypoxic environment.Hypoxia might stimulate the malignant potential of cancer cells.The purpose of our study was to clarify the significance of hypoxia in invasive breast ductal carcinoma by evaluating the expression of a hypoxic marker,namely carbonic anhydrase-9(CA-9).Methods The expression of CA-9 in the 100 samples was detected by non-biotin immunohistochemical method,the expression of positive cells for CA-9 was evaluated,and its association with histological grade,lymphatic metastasis,TNM stage and prognosis was assessed.Results The CA-9 expression was positive in 29(29.0%) of 100 invasive breast ductal carcinomas.CA-9 positive expression was signifcantly corresponding to lymph node metastasis(P=0.015),TNM stage(P=0.018) and overall survival rate(Log-rank=15.638;P=0.0001) or disease free survival rate(Log-rank=14.678;P=0.0001),but not to age(P=0.375),tumor size(P=0.288) and histological grade(P=0.526).CA-9 was an independent prognostic factor by multivariate survival analysis(P=0.002).Conclusions The expression of CA9 is strongly associated with neoplastic progression which suggests hypoxic microenvironment may play an important role in invasive breast ductal carcinoma.Hypoxia might be associated with aggressive tumor phenotype of invasive breast ductal carcinoma.The hypoxic marker CA-9 may be one of the useful prognostic indicators for breast cancer.
