摘  要:目的观察采用神经干/祖细胞(NSPC)和脐带间充质干细胞(MSCs)联合治疗对神经系统后遗症状的改善作用。方法选择3例以运动功能受损为主的神经系统疾病(病程>2年)病例,分别是遗传性共济失调、脊髓灰质炎后遗症和脑梗死后遗症,联合采用NSPC和脐带间充质细胞移植,每周1次,2例治疗4次,1例治疗2次。途径为经脑脊液(腰椎穿刺)、经静脉及损伤靶点定向注射,以世界神经病联合会国际合作共济失调量表(ICARS)评定运动功能的改变。结果每例患者的运动功能均有改善,治疗前ICARS平均评分为19.7,治疗后平均评分为14.7。副反应主要为在治疗后出现短暂的低热和头晕,均自行消失。从移植途径上看,脑脊液和损伤靶点处注射较静脉输注疗效明显。结论我们的结果表明,NSPC和MSCs细胞联合移植是安全的,对于神经系统疾病后遗运动功能障碍,至少短期内(1个月内)是改善作用的,但是确定的结论尚需在将来积累更多的样本,更长的观察期限和设立严格对照的研究后得出。Objective To evaluate the role of the neural stem/precursor cells(NSPC)and mesenchymal stem cells(MSCs) transplantation at the same time in improvement of neurological impairment in neurological diseases.Methods 3 neurological disease cases were selected with obvious motor system dysfunction,which were hereditary ataxia,sequela of poliomyelitis and brain infarction.Durations of all 3 cases were longer than 2 years.The therapy was transplantation of NSPC and MSCs at the same time through intrathecal injections,intravenous infusions and/or intraparenchymal transplantations.The improvement of motor function was confirmed by International Cooperatice Ataxia Rating Scale(ICARS).Results Clinically,two patients exhibited improvement in walking and one in the power of the involved upper extremity.The average ICARS scores were 19.7(24,13,22,respectively)before transplantation and 14.7(16,11,17,respectively)after transplantation.It seems intrathecal injections and intraparenchymal transplantations may have better improvement than intravenous infusion.The side-effect of neural stem cell transplantation in these 3 cases were low fever lasted for 2-4 days after therapy and slight dizziness.All of those signs could disappear without treatment.Conclusions While this observation highlighted NSPC and MSCs transplantation at the same time are safe and may improve motor function,at least in the following-up time(1 month) we investigated,it is still a matter of debate until studies including more samples,longer follow up with control groups will be carried out.
