老年病人一次多个牙拔除的体会

摘　　要：目的:通过对口腔科50岁以上老年人进行局麻下一次多个牙拔除的临床观察,研究适合我国国情的高龄病人的拔牙方法,以安全、有效的解除老年患者的牙病痛苦。方法:将暗示、安慰、音乐等心理治疗手段用于拔牙全过程,结合无痛麻醉、快速拔牙技术和随访等方法进行老年病人患牙的一次拔除。结果:50岁以上老年拔牙病人100例进行无监测局麻下拔牙,除4例病人在麻醉后出现短时间的头晕现象,2例出现心慌,2例麻醉不全而需短暂休息外,所有病人均顺利完成拔牙手术,术后个别病人自述术区疼痛,无其它不良反应。结论:高龄病人的拔牙,应除外病人正处于心血管病、糖尿病等全身性疾病的急性发作期,处于稳定期的病人都可以经受一次多个牙的拔除,但医生需有丰富的临床经验,并要取得病人及家属的绝对信任;此外,医院和医生应有应急处理的能力和条件,并应准备好急救物品,To explore a new method of multiple-tooth extraction for old patients based on evaluation of outcomes in 100 patients exceeding 50 years old. Methods: Effective anesthesia and quick operation, combined with psychotherapy, including suggestion, consotation, and tranquilizing music were implemented during the pro cedure. Results: All of the 100 cases tolerated the procedure well, with 4 patients complaining about transient diz ziness, 2 with tachycardia, 2 complaining pain after surgery. No other complications were observed. Conclusion: Multiple-tooth extraction can be implemented in the majority of elderly patients. However, it is our opinion that it is contra-indicated in those who are in the active stage of cardiovascular disease or diabetes.
