Glypican-3和AFP联合检测对原发性肝细胞癌的诊断意义

摘　　要：目的通过检测磷脂酰肌醇蛋白聚糖-3(Glypican-3,GPC3)和甲胎蛋白(AFP)在原发性肝细胞癌(HCC)患者中的表达情况,探讨其联合检测对于HCC的诊断意义。方法采用ELISA和免疫组化的方法检测了89例肝细胞癌患者和30例健康人群血清和组织中的GPC3和AFP蛋白的表达情况,并进一步分析了GPC3的表达与HCC患者临床病理特征的相关性。结果 GPC3和AFP联合检测对HCC的诊断敏感性和特异性分别为91.0%和93.3%,显著高于GPC3或AFP单独检测的敏感性和特异性(P<0.05);且对AFP阴性的HCC患者,GPC3表达的阳性率亦较高。GPC3蛋白的表达与HBsAg以及是否转移无关,而与Edmondson病例分级及肿瘤大小相关。结论联合检测GPC3和AFP可有效提高临床对于HCC的诊断水平。Objective By detecting the phosphatidylinositol protein gag-3(Glypican-3,GPC3) and alpha-fetoprotein(AFP) in hepatocellular carcinoma(HCC) patients to the expression,to explore the diagnostic significance of combined detection for HCC.Methods Using ELISA and immunohistochemical methods 89 cases of liver cancer patients and 30 healthy persons serum and tissues in the GPC3 and AFP protein expression,and further analysis of the expression of GPC3 and HCC in patients with correlation of clinicopathologic features.Results GPC3 and AFP combined detection in diagnosis of HCC sensitivity and specificity were 91% and 93.3%,significantly higher than that of GPC3 or AFP alone detection sensitivity and specificity(P<0.05);and AFP negative patients with HCC,the positive rate of GPC3 is higher.Expression of GPC3 protein and HBsAg and metastasis,and Edmondson case grading and tumor size.Conclusion The combined detection of GPC3 and AFP can effectively improve the clinical diagnosis level for HCC.
