乳腺Paget病的X线诊断(附32例分析)

摘　　要：目的:探讨乳腺Paget病的X线特征和诊断价值,提高对该病的认识及诊断准确率。材料与方法:回顾性分析32例乳腺Paget病的临床资料和X线影像,所有病例均经手术后病理证实。结果:32例女性乳腺Paget病患者年龄为27～76岁,平均52岁,临床分型:纯型9例,混合型12例,隐型11例。所有病例均为单侧,位于右乳16例,左乳16例。26例出现乳头乳晕皮肤增厚,6例出现乳头变形、凹陷,17例可见大导管征,13例可见乳头乳晕钙化,17例可见沿导管分布的钙化,29例可见乳腺实质内异常。结论:乳腺Paget病的X线表现具有一定特征性,主要表现为乳头乳晕改变、大导管征、钙化和乳内肿块。X线检查对于乳腺Paget病的诊断具有重要价值。对于纯型和混合型病例,X线摄影不仅能够显示乳头乳晕区的异常征象,也能够明确乳内的病变征象,还能发现一些尚未出现乳头乳晕改变的临床隐匿性病例。对典型病例,X线摄影可作出直接诊断。但对于隐型病例,确诊仍主要依据病理学检查,而X线检查能够明确乳内病变的部位和范围,为诊断和治疗方案的选择提供重要依据。Purpose:To investigate the X-ray features of Mammary Paget's Disease(MPD) in order to improve the diagnosis.Materials and Methods:The X-ray findings and clinical presentations of 32 patients with MPD confirmed by operation and pathology were analyzed retrospectively.Results:All patients were females and age raged from 27 to 76 years old(the mean age was 52 years old).They were all solitary lesions with 16 in the right breast and 16 in the left.These cases including simple type(9 cases),mixed type(12 cases) and occult type(11 cases).X-ray features included nipple and areola skin thickening(26 cases),nipple deformed and retraction(6 cases),catheter dilatation(17 cases),catheter calcification(13 cases),glandular substance abnormality(29 cases).Conclusion:MPD mainly displayed nipple and areola abnormality,catheter dilatation,calcification and mass.Their imaging findings have some characteristics.X-ray plays a significant role in diagnosis of MPD.
