卵巢癌中Twist和E-cadherin的表达及预后意义

摘　　要：目的探讨Twist及E-cadherin(E-cad)在卵巢癌中的表达及预后意义。方法选择具有完整临床病理资料及随访资料结果的卵巢癌50例,同时选取20例良性卵巢囊腺瘤组织和20例卵巢交界性囊腺瘤组织作为对照,采用免疫组织化学SP法分别检测Twist和E-cad的表达情况,分析其表达与临床病理特征及患者预后的关系。结果 Twist在卵巢癌中的表达显著高于良性卵巢囊腺瘤组织和交界性囊腺瘤组织(P<0.001);E-cad在卵巢癌组织中表达显著低于良性及交界性卵巢囊腺瘤组织(P<0.05)。Twist和E-cad在卵巢癌中表达呈显著负相关(r=-0.491,P<0.001);卵巢癌组织中Twist的表达与临床FIGO分期、组织学分级、腹水、淋巴结转移相关(P<0.05);Kaplan-Meier生存曲线分析显示患者术后生存率随Twist阳性表达呈现下降趋势,而随E-cad阳性表达显著上升;单因素预后分析提示Twist(P<0.001)、E-cad(P=0.042)表达异常、FIGO分期(P=0.001)、组织学分级(P=0.004)、腹水(P=0.003)、淋巴结转移(P<0.001)与预后相关。Cox多因素分析提示仅FIGO分期为独立危险因素(P=0.005)。结论卵巢癌中Twist与E-cad的异常表达与其侵袭转移密切相关,可能在卵巢癌的发生发展中起着重要作用,Twist和E-cad蛋白检测可作为临床上卵巢癌预后判断的参考指标之一。Objective To investigate the prognostic significance and expression of Twist and E-cadherin in ovarian carcinoma.Methods Fifty cases of surgically resected ovarian carcinoma with clinicopathologic and follow-up data were retrospectively reviewed,and 20 cases of ovarian borderline cystadenoma and 20 cases of ovarian benign cystadenoma were studied as a control at the same time.The expression of Twist and E-cadherin were detected by using immunohistochemical PV9000 method.The clinicopathologic characteristics and prognostic significance were analyzed.Results The positive expression rates of Twist was higher in ovarian carcinoma than borderline and benign cystadenoma(P<0.001).The positive expression rates of E-cadherin was lower in ovarian carcinoma than borderline and benign cystadenoma(P<0.001).There was a significant negative correlation between the expression of Twist and E-cadherin in ovarian carcinoma(r=-0.491,P<0.001).The expression of Twist and E-cadherin had correlation with clinical pathologic stage,histological grade,ascites,lymph node metastasis(P<0.05).Kaplan-Meier analysis showed postoperative survival rate of patients with Twist positive expression has a downward trend,while the positive expression of E-cadherin increased significantly.Univariate analysis showed that the expression of Twist(P<0.001),the expression of E-cadherin(P=0.042),clinical pathologic stage(P=0.001),the grade of histological grade(P=0.004),ascites(P=0.003),lymph node metastasis(P<0.001)represented important prognostic factors.Multivariate analysis showed that only clinical pathologic stage(P=0.005)was an independent prognostic factor.Conclusions Twist high expression and E-cadherin low expression in ovarian carcinoma is closely related to the invasion and metastasis,and maybe play an important role in the development of ovarian carcinoma and represents a useful criterion in prognostic categorization of ovarian carcinoma for clinic.
