Survivin和Livin蛋白在非小细胞肺癌患者痰液中的表达及临床意义

摘　　要：目的观察非小细胞肺癌(NSCLC)患者痰液中Survivin和Livin蛋白的表达情况以及两者之间的关系,以探讨痰液免疫细胞化学染色在肺癌辅助诊断中的应用。方法采用改良Saccomanno法收集68例经活检或手术确诊的NSCLC患者的痰液标本,15例肺纤维化(对照组)和10例正常人(对照组)痰液标本,采用免疫细胞化学染色法检测痰液中Survivin和Livin蛋白的表达情况,分析这两种蛋白在NSCLC患者痰液中的表达水平,与肺癌的组织类型、分化程度、性别和年龄之间的关系以及两种蛋白表达相关性。结果 NSCLC中Survivin和Livin蛋白的表达阳性率分别为55.9%(38/68)和67.6%(46/68),均显著高于正常组及肺纤维化组(P<0.05),且其表达均与细胞学类型、肿瘤分化程度、性别和年龄无关。Survivin与Livin蛋白之间的表达无相关性(rs=0.053,P>0.05)。结论 Survivin和Livin蛋白可能与NSCLC的发生发展有关。通过痰液免疫细胞化学染色检测这两种蛋白可能为非小细胞肺癌的诊断提供一个新的、安全无创的辅助检查方法。Objective To study the expression of Survivin and Livin proteins in the sputum sample of non-small-cell lung cancer(NSCLC),and analyze the correlation of survivin expression to Livin expression.To evaluate the feasibility of immunocytochemical assay in sputum sample.Methods The expression of Survivin and Livin proteins was evaluated by immunocytochemical assay in the sputum sample of 68 NSCLC proved by operation or biopsy,15 lung lesions(control group)and 10 normal samples(control group).The sputum was collected and pretreated by improved Saccomanno′s concentration method.The relationships between the two proteins,pathologic category,degree of differentiation,gender and age were compared.Results The positive smear rates of Survivin and Livin protein were 55.9%(38/68)and 67.6%(46/68).The expression of Survivin and Livin protein were significantly elevated in NSCLC sputum samples compared with lung lesions and normal samples(P<0.05).The expression of Survivin and Livin expression was not correlated to pathologic category,degree of differentiation,gender and age(P>0.05).The expression of Survivin had no relationship with Livin expression(rs=0.053,P>0.05).Conclusions Survivin and Livin proteins may be valuable for diagnosing NSCLC in sputum sample.Detecting Survivin and Livin in sputum by immunocytochemical assay might be used as one of the new and easy methods for diagnosis of NSCLC.
