摘　　要：目的探讨通过腹壁下动脉置管将化疗药物灌注入治疗盆腔恶性肿瘤的临床疗效和不良反应情况。方法将2010年2月～2011年12月在某科治疗的48例盆腔恶性肿瘤患者随机分为灌注组和非灌注组两组,其中灌注组采用腹壁下动脉置管灌注化疗方式治疗,非灌注组采取普通动脉化疗方式治疗,比较两种不同化疗方式的临床疗效,并发症情况和血清中CA125变化情况。结果近期疗效灌注组的总有效率为91.67%,显著高于非灌注组的79.16%,P﹤0.05;灌注组并发症状恶心呕吐、腹痛腹泻、白细胞减少、发热的发生情况和近期复发率均显著低于非灌注组,P﹤0.05;治疗后灌注组CA125水平下降超过50%,共占55.56%,显著高于非灌注组的29.41%,P﹤0.05。结论经腹壁下动脉置管灌注治疗盆腔恶性肿瘤疗效显著,具有良好的科学性、可操作性及推广价值。OBJECTIVE To investigate the clinical efficacy and side effects of arterial tube perfusion through the abdominal wall in the treatment of pelvic malignancies.METHODS 48 cases of surgical treatment in patients with pelvic malignancies from February 2010 to December 2011 were randomly divided into the perfusion group and the non-reperfusion group.The reperfusion group was taken with inferior epigastric artery catheter infusion chemotherapy treatment,and the non-perfusion group was taken with the ordinary arterial chemotherapy treatment.The clinical efficacy,complications and serum CA125 changes of the two different chemotherapies were compared.RESULTS The total effective rate of recent efficacy perfusion group was 91.67%,significantly higher than the 79.16% of the non-reperfusion group(P﹤0.05).The concurrent symptoms of nausea and vomiting,abdominal pain,diarrhea,leukopenia,fever,and the recent recurrence rate in perfusion group were significantly lower than non-reperfusion group(P﹤0.05).The CA125 level dropped by more than 50 percent after treatment,accounted for 55.56%,significantly higher than 29.41% of the non-reperfusion group(P﹤0.05).CONCLUSION The clinical effect of inferior epigastric artery catheter instillation for the treatment of pelvic malignancies is significant,it is with a good scientific,operational and promotional value.
