3.5mm切口手法碎核与超声乳化在硬核白内障手术效果的比较

摘　　要：目的比较硬核白内障在3.5mm切口手法碎核白内障摘除与超声乳化白内障摘除术后视力恢复的特点及影响因素。方法 106例Ⅳ级核白内障患者,随机分为不同术式的2组:超声乳化组和3.5mm切口手法碎核组。比较2组患者术前及术后不同时期的最佳矫正视力、角膜散光值、角膜内皮细胞计数及角膜水肿程度。结果 2组患者术后1d最佳矫正视力平均值的差异有统计学意义(P<0.01),术后1周、3月2组间差异无统计学意义(P>0.05);2组患者角膜内皮细胞丢失率术后不同时间比较差异均无统计学意义(P>0.05);术后1d1～3级角膜水肿的发生率差异有统计学意义(P<0.05),术后1周差异无统计学意义(P>0.05);超声乳化组在术后1周及术后3月的散光值与术前比较减小,其差异有统计学意义(P<0.05)。结论本研究所探讨的2种手术方式,硬核白内障均可以有效实施。Objective To assess the safety and effect of different methods of cataract surgery for hard nuclear cataracts. Methods A total of 106 cases with hard nuclear cataract were randomly divided into two groups. The average best corrected visual acuity, the corneal astigmatism, corneal endothelial cell and the corneal edema degree pre-operation and post-operation was observed and analyzed between the two groups. Results There was a significant difference between the two groups in average best corrected visual acuity on 1st day after operation(P<0.01), but there was no significant difference between the two groups 1 week and 3 months after operation(P>0.05). There was no statistical difference in the loss rate of corneal endothelial cell.The incidences of corneal edema of the two groups were significantly different on 1st day after the surgery(P<0.05). The corneal astigmatism 1 week and 3 months after operation was significantly decreased(P<0.05) in phacoemulsification group. Conclusions In this study, the two operation methods are both effectively implemented for hard nuclear cataracts.
