白蛋白、C反应蛋白、乳酸脱氢酶对多发性骨髓瘤临床分期和预后的指导意义

摘　　要：目的探讨白蛋白(Alb)、C反应蛋白(CRP)、乳酸脱氢酶(LDH)对多发性骨髓瘤(MM)临床分期及预后的指导意义。方法按照Durie-Saimon(DS)分期标准将MM患者分为DSⅠ～Ⅲ期,比较各期患者间Alb、CRP、LDH水平的差异,分析各指标与DS分期的相关性。结果除DSⅠ期与Ⅱ期的骨髓浆细胞百分数(BMPC)及LDH水平差异无统计学意义(P>0.05),其余各期指标间两两比较差异均有统计学意义(P<0.05)。β2微球蛋白(β2-MG)、BMPC、CRP、LDH与DS分期均呈正相关(P<0.05),β2-MG、BMPC与CRP、LDH均呈正相关(P<0.05),β2-MG与BMPC亦呈正相关(P<0.05),Alb与DS分期、β2-MG、BMPC均呈负相关(P<0.05)。结论 Alb、CRP、LDH水平可间接反映MM的肿瘤负荷、分期及预后,为临床选择治疗方案提供实验室数据支持。Objective To explore the guiding significance of albumin(Alb),C-reactive protein(CRP),lactate dehydrogenase(LDH) in clinical stage and prognosis of multiple myeloma(MM).Methods The patients with MM were divided into stage Ⅰ-Ⅲ according to durie-saimon(DS) staging criteria.The levels of Alb,CRP,LDH were compared between the patients in different stages.The correlation betweent each index and DS staging was analyzed.Results Except for bone marrow plasma cell(BMPC) percentages and LDH between stage Ⅰand Ⅱ,there were significant differences of every index between different stage(P<0.05).β2-microglobulin(β2-MG),BMPC,CRP,LDH showed a positive correlation with DS staging(P<0.05),and β2-MG,BMPC showed a positive correlation with CRP and LDH(P<0.05).There was a positive correlation between β2-MG and BMPC(P<0.05),and a negative correlation between Alb and DS staging,β2-MG,BMPC(P<0.05).Conclusions The levels of Alb,CRP,LDH can indirectly reflect tumor burden,staging and prognosis of MM,and it can provide laboratory data support for the choice of clinical therapy.
