铜陵市城区居民2000-2009年恶性肿瘤死因分析

摘　　要：目的了解铜陵市城区居民2000-2009年恶性肿瘤死亡状况,为铜陵市恶性肿瘤防制工作提供参考依据。方法应用死亡率、标化死亡率、潜在减寿年数(YPLL)和潜在减寿率(YPLL率)等指标分析居民恶性肿瘤死亡情况。结果十年间,恶性肿瘤死亡率呈波浪式上升趋势(χ2=17.51,P<0.01)。恶性肿瘤年均粗死亡率144.40/10万,标化死亡率143.65/10万,YPLL率11.3‰,均居全死因首位。在恶性肿瘤中,肺癌死亡率为36.64/10万,占25.38%,居首位,其次为胃癌、食管癌、肝癌、结直肠癌。前五位恶性肿瘤YPLL率顺位为肺癌、胃癌、肝癌、食管癌、白血病。男性肝癌、女性乳腺癌YPLL率顺位与死亡率顺位有较大变化。结论恶性肿瘤已成为铜陵市城区居民重要死因,肺癌、胃癌、男性肝癌、女性乳腺癌、青少年白血病是铜陵市城区今后恶性肿瘤防治工作的重点。Objective To investigate the death cause of malignant tumors of urban inhabitants in Tongling and to provide the basis for preventing and controlling malignant tumors.Methods Mortality rate,standardized mortality rate,years of potential life lost(YPLL) and YPLL rate were used to analyse death cause of malignant tumors.Results In the decade,the mortality rate of malignant tumors was wave-like rise trend(χ2=17.51,P<0.01).The annual average mortality rate of malignant tumors was 144.4/105,with standardized mortality rate of 143.65/105 and YPLL rate of 11.3‰.Among the malignant tumors,lung cancer took the lead with a mortality rate of 36.64/105,accounted for 25.38%.The next sequences were as follow:stomach cancer,hepatitis cancer,oesophagus cancer,and intestine cancer.The top five malignant tumors with highest YPLL rate were lung cancer,followed by stomach cancer,hepatitis cancer,oesophagus cancer and leukemia.The sequences of YPLL rate of male hepatitis cancer and female breast cancer significantly differ from the sequences of mortality rate.Conclusion Malignant tumor has become the major cause of death in Tongling city.In future,prevention and control of lung cancer,stomach cancer,male hepatitis cancer,female breast cancer and adolescent leukemia will be focused in the urban area.
