摘  要:目的探讨精神分裂症认知功能患者与儿茶酚胺氧位甲基转移酶(COMT)基因rs4680多态性位点的关系。方法采用威斯康星分类测验(WCST)对2008年7月至2010年6月在济宁市精神病防治院住院符合美国精神障碍诊断与统计手册第四版(DSM-Ⅳ)诊断标准的167例精神分裂症患者与98名健康对照组进行电脑不同颜色、形状、数量的模块卡片推理归类,并对完成情况进行记录,评估其认知功能。并应用限制性片段长度多态性的方法进行COMT基因rs4680多态性位点分析。采用卡方检验和方差分析进行统计分析。结果精神分裂症患者(169例)和健康对照者(98例)之间COMT基因rs4680多态性基因型及等位基因的分布差异均无统计学意义(χ2=4.40,P=0.110;χ2=0.726,P=0.394)。精神分裂症患者中不同基因型组之间WCST持续性错误数得分差异具有统计学意义(F=11.496,P<0.01),G/G基因型患者持续性错误数成绩高于G/A基因型(分别为56.20±15.11,43.44±15.35),差异具有统计学意义(F=11.496,P<0.05)。不同基因型组间WSCT其他指标的差异均无统计学意义(F=0.125,F=0.535,F=0.178,F=1.861,F=0.102,F=1.305,P>0.05)。结论未发现COMT基因rs4680多态性与精神分裂症存在关联;COMT基因与精神分裂症患者认知功能具有相关性,G/G基因型患者认知损害较明显。Objective To study the relationship between rs4680 polymorphism of catechol-O-methyl transferase gene( COMT) and the cognitive function of patients with schizophrenia. Methods Schizophrenia was diagnosed in 167 patients in Mental Hospital of Jining from July 2008 to June 2010 according to the diagnostic and statistical manual of mental disorders versionⅣ( DSM-Ⅳ) diagnostic criteria. For the control group,98 normal volunteers were chosen. Wisconsin card sorting test( WCST) was used to evaluate the cognitive function,all the cases were given module card reasoning classification test of different color,shape,number,and accomplishment were recorded. Polymerase chain reaction( PCR) and restriction fragment length polymorphism( RFLP) techniques were performed for COMT rs4680 polymorphism. The chi-square test and analysis of variance was used for statistical analysis. Results When the schizophrenia group( 167 cases) and the normal control group( 98 cases) were compared,no difference was seen on the COMT genotype or the allele frequencies( χ2= 4. 40,P = 0. 110; χ2= 0. 726,P = 0. 394). There were differences of continuity error number on different genotype group of schizophrenia patients( F = 11. 496,P < 0. 01). G / G group was higher than G / A group( 56. 20 ± 15. 11 and 43. 44 ± 15. 35 respectively,P < 0. 05). There was no difference in other items of WCST on different genotype group of schizophrenia patients( F = 0. 125,F = 0. 535,F =0. 178,F = 1. 861,F = 0. 102,F = 1. 305,P > 0. 05). Conclusions There is no association between the rs4680 polymorphism of COMT gene and schizophrenia. However,there might be association between rs4680polymorphism and some cognitive function. The cognitive impairment of G / G genotype is serious.
