老年宫颈癌综合治疗中监测贫血各项指标变化的临床意义

摘　　要：目的探讨监测贫血各项指标的变化对老年宫颈癌患者远近期疗效的影响及临床意义。方法对象选择2005年8月至2012年8月在湖北省荆州市中心医院肿瘤科住院的宫颈癌患者,为Ⅱa~Ⅲb期,共346例。按血红蛋白(Hb)浓度的高低分为正常组、轻度贫血组、中度贫血组、重度贫血组。观察放、化疗前后血液各项指标的变化,同时分析不同Hb水平与患者肿瘤的疗效或5年生存率之间的关系。结果Ⅱ期宫颈癌患者放、化疗前Hb、血清铁、总铁结合力与放、化疗后均有统计学差异(P<0.05);Ⅲ期宫颈癌患者放、化疗前红细胞计数、Hb、血清铁、总铁结合力与放、化疗后均有统计学差异(P<0.05)。放、化疗前患者Hb水平与患者局部肿瘤的疗效成正比,Hb水平越低,疗效越差。而且随着放、化疗前贫血程度的加重,5年生存率也会逐渐降低。结论在放、化疗前后监测血液各项指标的变化,有助于判断宫颈癌综合治疗的疗效,为临床治疗提供更加可靠的方案。Objective To investigate the effects of anemia indexes on the short-and long-term efficacy of chemotherapy or radiotherapy in elderly patients with cervical cancer.Methods From August 2005 to August 2012,346 patients with Ⅱa-Ⅲb cervical cancer in Jingzhou Central Hospital were enrolled in this study.According to the level of hemoglobin the patients were divided into normal group,mild anemia group,moderate anemia group,severe anemia group.The changes of indexes of blood were observed before and after chemotherapy or radiotherapy.At the same time,the relationship of the levels of hemoglobin before radiotherapy or chemotherapy with the efficacy and five-year survival rate were analyzed.Results The levels of hemoglobin,serum iron,total iron binding capacity showed significant differences before and after chemotherapy or radiotherapy in the patients with Ⅱa-Ⅲb cervical carcinoma(P < 0.05);The level of hemoglobin before treatment was significantly related with the efficacy and five-year survival rate.Conclusions Monitoring the changes in the index of blood before and after chemotherapy or radiotherapy may help judge the curative effect of comprehensive treatment of cervical cancer.
